Doctor Recommendation Needed - Philly and NJ

Hello -- I'm VERY new to this forum and just was diagnosed yesterday afternoon, but don't have any other information other than I need to see a breast surgeon. As you can imagine, I am scared and in shock.

I live in Central NJ, and am seeing a breast surgeon in E. Windsor (Rachel Dultz), for a consult, but I really am comfortable with Philly (UPENN) as I had a hysterectomy there in 2009 as well as an ablation and urethra surgery.

Could anyone give me feedback on the Rena Rown Breast Center in Philly? Also, are there any good breast specialists in NJ? Again, I'm more partial to Philly and want the best care.

 Any suggestions would be most helpful.

    I am not familiar with UPENN (I have heard good things about them) but LOVE my surgeon who practices out of Einstein.  Of course the only surgery I've had is port implantation (twice due to staph infection) but my surgeon was my first contact (referred by by GYN). 

    Due to my circumstances, my husband was the main contact in the beginning and she was great with him. She is very compassionate and caring.  I had to see her partner for my second port because she was out on medical leave (knee replacement) and she was just as good. 

    You might want to check out Robert Wood Johnson in New Brunswick - that would be close to you (I don't know anything about them but they have an NCI recommended Cancer Center).

    I am going to UPENN.  I don't really want to be there, but if I have to be doing this somewhere, I feel I am at the best place I can be.  I am less than 10 miles outside of Philly, so it is an easy commute for me.  I will say that I am getting the same treatment others are getting in all the different places they are going, so there seem to be very much standards of care.  I've also read someone on here from Lancaster has treatment locally, but her docs talk with Kevin Fox at UPENN.  I believe he is the head of the Rena Rowan Center, so that is a possibility for you.  I am seeing a very young onc there, I am pretty sure she is only in her mid 30's, but I am very comfortable with her.  It also makes me feel good to know she is very much up to date on everything.  Good luck with your decisions and if there is anything I can tell you about UPENN, pm me.  It is a beautiful new building and everyone there is very compassionate and great to deal with.

    I have seen Kevin Fox twice at UPENN, but I was treated locally in central Jersey.  I also spoke with wo onclogists in the New Brunswick area.  Another local option is Fox Chase.

    I only looked into Penn for reconstruction. I had my bmx at Lankaneu with Dr Jennifer Sabol. I can't say enough good things about her. She was great both during diagnosis and surgery as well as afterwards. All she does is breast surgery. Lankaneu is also where Dr Weiss is located and I had a great consult with her also. I would highly recommend at least a consult. They have a great (newly constructed breast center) and have great patient advocates. Also if surgery is necessary I have not found better nursing staff

    Chrys:  I am so sorry you had to join us - it is such a shock but it will get better in time.  I live in Montgomery NJ (just outside Princeton) and we are very fortunate to have some great doctors in this area.   Dr. Angela Lanfranchi (breast surgeon, Steeplechase Cancer Center in Somerville) diagnosed me and I credit her with saving my life (she did the surgical biopsy and later mastectomy).  She is a straight shooter and tells you like it is.  Dr Kathleen Toomey (also at Steeplechase) is my oncologist and I just love her - she is amazing:  very knowledgable, up to date, warm, direct and kind.  I had a Stage IIb diagnosis, had a 2nd opinion at Sloan K. and was very happy that I found this wonderful local team and didn't have to travel during treatment.  Today Dr. Lanfranchi is not in network for my insurance - which may have had an impact on my choice of surgeon (but I am hearing that all/most of the NJ breast surgeons opted out of network).     Chrys:......I had man options in Philly since I am in the suburbs..............I chose the Delaware County Memorial Hospital, in Drexel HIll, Pa...............It is a Regional Cancer Center of Fox Chase of Philadelphia............It is a 1 stop place..........everything is done there, and nothing leaves the building........I had my pathology results back in 2 days, and the final in 2 more days...........My Dr. actually carried my biopsy from the room he did it in, right to the Pathology Dept. in the hospital.........He also did my biopsy, and I know a lot of women whose biopsies wsere done by Radiologists........

    My surgeon was the Chief of the Breast Cancer Center.........wonderful Dr., my MO, and my RO, are great too...........Especially my RO.  the techs are fantastic, and everyone is so caring...............Probably a little far for you to travel, but just wanted to let you know how pleased I was with my care...............

    You may be interested to know that studies show that patients often have better outcomes when their operations are performed by surgeons who do a high volume of the same operation. Such surgeons are often found at comprehensive cancer centers that are recognized for their extensive experience in treating patients with cancer.     Thank you all for your kind replies!

    I had my consult today in my area -- I have IDC, and a lumpectomy with sentinal node biopsy is recommended.  The appointment with UPenn actually got moved to June 1st, because they needed all my records and my pathology slides by TOMORROW for me to keep my appointment. I have everything, but the slides. Frown  

    In the interim, I called Sloan Kettering this afternoon and faxed over my reports, they can fit me in on May 31st  -- one day earlier than UPenn. Ugh...I'm so scared to wait for the consult and then have to possibly wait for the surgery, which could be another month from now. We also had a vacation planned months ago for the end of June.

    I don't know what to do; have surgery locally with a non-NCI surgeon/center or wait over a week to get in to Sloan.  Decisons, decsions....I feel sick mentally and physically. :(

    I live just outside Princeton and Rachel Dultz is a great doctor.. I didn't go to her, but I hear great things... I went to Susan McManus in Somerset... Another option is Dr Kearney at the Cancer Institute in New Brunswick... PM me if you have questions... where do you live? I don't think you want to go too far.

    I know how difficult all of this can be for you.  I would suggest that you wait to see both doctors before you make any decision.  I have found that feeling comfortable with your doctors is very important.  You will be together for many years, and must feel that they sincerely care about you and that you can discuss anything at all with them. 

    The best medical center and the best doctor is not the way to decide - the one that you feel compatible with who has the time to patiently listen to you and thoroughly answer your questions in my opinon is the most important factor. 

    The diagnosis of breast cancer is scary enough and you will need doctors who understand the emotional (as well as the physical) toll this disease takes.  A competent, caring compassionate doctor is ver important.  So if you need to go to 2 or 3 doctors until you find the one that is right for you - then do so.  I did and it made all the difference in the world!

    CHRYS... Bigger isn't necessarily better... I have friends who went to MSK and were very unhappy... I agree to like AND trust your doctor.. and do your homework... and remember having a lumpectomy and SNB is very common... once you get your pathology report you can always change your course of action. I think your BS needs to be knowledgable and competent (as Rachel is) but you will spend far more time with your medical oncologist... and if you need chemo will be there often, so you may not want to be travelling a lot for that.

    I went to one BS and then one MO, and now that I am done with chemo and need to decide between radiation (docotor preference) or mastectomy (my preference) I have been meeting a lot of other doctors... take your time and make good decisions...

    A close friend of mine went to Susan McManus and couldn't say enough good things about her.



    I'm actually on the other side of Phila (southwest) but my lymphedema physiatrist is at the Rowan BC, and I have had a good experience there. My lymphedema therapist is also with Penn, but at a suburban office near me.

    Thanks everyone!  BTW, I live in Princeton......I have NO CLUE what to do -- I don't. There is just too much information and I don't know what research to look for. I just thought that MSK would be the place to go, because it's bigger and #2 in the country.

    I really want to have the surgery immediately to see then WHAT I am dealing with --but then  where the hell do I find a medical oncologist?? What do I do?? I am sick of dealing with these consults and waiting and then not know what to do. :(

    Also... if you went to the highly rearded Cancer Institute (a lot more convenient than Philly or NYC) you could see Dr Kearney (BS) and Dr Toppmeyer (MO) and RO if necessary all in one location... My friend goes there and loves them.... Taking a few extra days now can save you regrets later... a few days isn't going to change the outcome and gives you time to plan it out...

    HI I also had a less than great experience at Memorial Sloan in Basking Ridge with their breast cancer surgeon.

    Ended up in Weschester New York after Fox Chase and a another surgeon in Philly.

    I am currently being treated at UPenn.  My surgeon is Dr. Brian Zernicki (may have spelled that wrong) and my onco is Dr. Kevin Fox.  Had a lumpectomy 5 weeks ago, Dr. Ziernicki was a straight shooter.  Not a great bedside manner, but told it like it is.  I like that.  I met with Dr. Kevin Fox for the first time on Monday, he was awesome.  He really took the time to sit and talk with me.

    I also had Dr Lanfranchi and was pleased with her.  Others have commented that her bedside manner is missing warmth but I didn't have a problem.   Then she went out of network with Somerset Medical Center (which came back in after one year but I'm not sure if she's back or not).  If necessary (and I hope not) and I need a BS again Dr Susan McManus is highly recommended by many people in my local support group.  And the Cancer Institute of NJ in New Brunswick is said to be top-rated.  So if I need it again, it's Dr McManus and CINJ. 

    You are right though, it's sometime so much information.

    edited to add another answer for you, Dr Richard Fein on Veronica Avenue in Somerset is highly regarded as medical oncologist.

    Hello Chrys:  I go to Rena Rowen, have for several years, Dr Kevin Fox is great, and the staff is the best.  Surgeon Dr Brian Czernicki did a great job also.  Would highly recommend them.  If you have any questions or would like to talk, just let me know.  best to you..

    Hi Chrys..........I was at a Regional Cancer Center, in Drexel Hill, Pa.............My surgeon was the most skilled, and the nicest person I had ever met..........He was so compassionate, kind and caring.......I liked him from the moment I met him.............I had my diagnosis on Feb 15th after a repeat mammogram......he had me set up for my biopsy on the 21st. of Feb, and had my results in 2 days.......met for 2 hours with my children, and set up surgery for March 14th, he was wonderful, and in between the biopsy and surgery........he set me up with appts. with RO, and an MO, and they are wonderful as well..............He was wonderful, and I do belive the only reason I got through all the "crap" of cancer, was I had a fantastic "team" of Dr.'s taking care of me..........Everything went fast, and here I am just 2 weeks away from being finished Rads.

    I wish you luck, and do remember, sometimes the big places are not the best.........My hospital is associated with Fox Chase Cancer Center in Philly, ...........but I don't have all the big hospital crap to deal with...........sometimes in those places your just a number...........where i went I felt like a "person".................good luck

    I travel three hours (one way) to be treated by Dr. Czerniecki at UPENN, and my surgery is scheduled for Monday.  I think he is very thorough, very kind, and I wouldn't hesitate to recommend him.

    Oh, I forgot to say my breast surgeon.  It is Dr. Julia Tchou at UPENN.   Haven't had surgery yet, it's scheduled for July 27, but I LOVE her.  Also very kind and very thorough and tells it like it is.  I did see she was on Phila. Magazine as best breast surgeon in Philly.  The NP's recommended her also, which is always a very good sign.  Having reconstruction at the same time with Dr. Liza Wu.   Also recommeded by NP's and Dr. Tchou.  Can't tell you how  things went yet though, but feel very confident and comfortable with them.

    I must admit, I am a UPENN alum and have many family connections with UPENN, both as alums and employees.  My son graduated from there last year as the latest alum.  When I knew I had breast cancer, there wasn't a shadow of  a chance that I was going any where else.  I am very confident and comfortable there.

    What really matters I think is not how small, how large, or how well known a surgeon, or your cancer team is..........I think what matters is how you feel about them personally..........you have to be happy with them, and feel the confidence in your team.........if you have any doubts just go somewhere else......its that simple...............I need to be able to connect, and communicate with my health care people,and if I can't, then I move on............I was ready to run to the biggest, and what was considered the best places in Philly to go..........I found my "special people" in a Regional Cancer Center connected with Fox Chase.......I didn't have to go to the "big building" to get superior care.......I got it 15 minutes from my home........I am a very appy patient.

    Hi, i am being treated at MSK. I see my oncologist at Basking Ridge and got chemo there. All my other doctors I see at the main hospital in manhattan. I am happy with the care that I received.

    Chrys---I went to 3 different drs.i live in old bridge NJ...i went to the jacqueline Wiletz breast center in Monmouth Medical Center in Long Branch NJ.Dr. Debra Camal is my BS..it took 3 different drs.to show me she is the BEST.she works together with onco,RO and internist.all the best....shy dont you give her a try????if you want any info pm me.she is the top BS at the center and worked very hard to get where she is today.she is young,cheerful,gives you as much time as you need...and she is a FEMALE.

    I was a ways into my dx when i went to Fox Chase Cancer Center in north PHILA. 

    I'll preface by saying I don't care what sex, color, religious affiliation my dr is, as long as he/she is the best fit for me 

     My ps, Dr Neal Tophan is the most awsome ps, and I interviewd three before finding the right one.  He is kind, compassionate and oh so skilled. I just cant say enough about him.  Wonderful man- and you should see his work. 

    Had my mastectomy done by Dr Bleicher, and did like him too, He is an oncology surgeon.

    It was a bit of a drive (about 1 1/2hr each way), but well worth it.  It may be easier to get to since it is north of the city.

    My FIL was treated there as well, and the family was very happy with his care.

    Hi Chrys 23,

         I first went to Dr. Dultz. I had my lumpectomy there, but I was not pleased with the overall treatment or the analysis of the pathology report.  I then went to Dr. Anne Rosenberg in Philly, who operates at Thomas Jefferson, whom I liked a lot more, but she doesn't take insurance and doesn't work with a cancer team, athough Jefferson does have a Cancer Center and can offer that option.  I've also heard good things about Beth Dupree.  

        You sound like you're really in pain.  I live in Trenton, and am recovering from a BMX.  Do you have friends who've been through this?  If not, would you like to sit down with a cup of coffee and talk?  I'm nearby and I have time.  Email me privately at sleemanning@ gmail.com - if you want.

    Crystal... You have a lot of suggestions and can't possibly see them all.. I would read all suggestions, see which plan makes the best sense for you and go forward... Remember each doctor is not a one size fits all... I would guess no doctor is loved by all, for one reason or another. And always remember that you can always change doctors, especially the BS... I think once surgery is over (whoever you shoose to use), choosing your oncologist is more important since he/she is the one to plan the future treatment... Its kind of a no-brainer to have the cancer removed, but the following treatments vary greatly, and you have to trust your oncologist 100%.

    Thanks everyone -- I appreciate all the suggestions.  I've sent all my paperwork, films and pathology slides to Rena Rowan BCC in Philadelphia. My appointment is June 1st (bumped from this Wednesday because my slides weren't in in time here in Princeton).  I still feel the waiting is horrible, but c'est la vie. It's one more week and if anyone cancels, they will move up my appointment (I doubt that will happen though).

    I have family in Philly (My BIL & SIL) and if I choose treatment in Philly, they have graciously stated I can stay with them so that I don't have to travel back and forth to Princeton in the event Chemo should ever be needed down the line.

    Tried to get in to see other places, even here in NJ -- but the wait time was end of June.

    Good luck.  I was seen there for a consultation two times and had a good experience there.  I had my chemo locally and see doctors here (though not as often now) for follow-up.
