How to extend SDI?
My Dr put in 4/10 as my date to return to work and told me that he can extend it if I wanted to.
Does anyone know where I can find the forms for both my Dr and myself to file for extended SDI? I can't find it on their website.

Are you asking about Short Term Disability Insurance or Social Security Disability Insurance or California State Disability?
The Short Term Disability would be through your employer, and usually covers up to six months, going to Long Term Disability coverage (if they offer it). Your human resources should have the forms and information.
Social Security Disability is long term based on updates from your healthcare providers.
If you are asking about State Disability; I can't answer as I don't reside in California. But someone should be along that does know who to contact. -

Maybe making the forms unavailable is a way the gov'ner is trying to balance the budget. (Sorry, really sick joke.)

CA SDI will send you an extention form in the mail when your time is up.

You will have 20 days to complete the form and return it to your doctor.

I just rec'd the Disability Insurance form from EDD of CA office requesting my Medical information. Do you know why do I need to get my doctor sign off on the form again when I'm currently receiving disability check already? I tried to call EDD office but not able to get through and tried to search on this sites I cannot find any relate to this subject. Any one can help is greatly appreciated.

I had to resubmit a few times while on SDI -- they just want to make sure you're STILL disabled. Just go ahead and have your doctor fill out the form and then send it in.
Tip on getting through the EDD phone line -- physically visit your local EDD office and they can put you through to a real person with very little delay. Otherwise, if you call from home plan to spend 45 minutes ...and possibly have the phone go dead.
