Hi Guys,
I have not been on the lymphedema boards very much, so I have no idea if this topic is covered elsewhere, but here goes.
I have been wrapping practically 24/7 for about 2 1/2 months now. A couple of weeks ago I started experiencing pain in the web of my hand between thumb and index finger. When I open that area, I can see what looks like two inflamed tendons or something. I thought maybe my stockinette was not lying flat or the bandages were too tight. Now, no matter how carefully I wrap, my hand hurts terribly. I am due to get new custom wraps soon, but my daytime wraps have not even been ordered yet. Any ideas on how I could alleviate this pain?

I hope Binney comes along to help: it sounds like you really chafed your web space, and you do want to avoid that as it could damage your skin and the lymphatics.
Binney has recommended padding the area with artiflex, or some soft foam, and she recommends corn starch when the chafe comes from garments.
When I've chafed, I've sometimes applied a cut piece of a telfa pad to the area with some bacitracin ointment to make it stick, and put a wrap of artiflex over the area before I wrap.
Since I often chafe in that area, I put a piece of soft foam in there before I wrap.
Now, I'm in a cast, and actually really wish I could wrap! Sad, no?

thank you kira.... I am so sorry to hear you're in a cast! That really bites! I don't think I'm chaffed... it seems like something that is taking place under the skin. Padding might help, though. I'm just scared if I add something more, it will make it worse. I'm gonna try it, though.

MissS, what a bummer!
I'm wondering how it's working to add some padding? Hope you're seeing some improvement in the pain level. I have trouble with pain in that area sometimes too, and do find a bit of extra padding helps (I use a square of Artiflex over the area. If you're wrapping your fingers with gauze you might also try wrapping the second wrap (the one for thumb and pinkie, y'know?) backwards -- start it going around your wrist in the opposite direction from what you've been doing. It's a little strange at first, but worth a try. For some reason that really takes both pain and swelling down in the area for me. (Be sure to keep your fingers and thumb well extended and separated as you're wrapping so you're not pulling the thumb inward with a too-tight wrap.)
My day gloves also cause this kind of pain, and what worked for me is to have my fitter measure around my palm VERY LOOSELY. That keep my thumb from having to work so hard to stay in a relaxed position -- a tight fit around the palm pulls in at the sides and makes my thumb sort of "collapse" into my palm in an unnatural position. If you have a lot of swelling in your palm or the back of your hard you could then use a Swell Spot to help with that instead of trying to get all the compression from a too-tight glove.

I have at times developed painful "nodules" at pressure points, that went away after extra padding and care.
If you can give that area a break, or pad, hopefully those irritated tendons will resolve.
Pain is NOT good in LE.

I have the same issue = I get a tender spot between my thumb and pointer finger because of the seams on my gauntlets. I use the micropore tape, and also, the liquid bandage stuff (can't remember the name - sorry!) works well, also. -
ank you so much for your replies. I'm wrapping with some extra padding right now so I will let you know. I also think the tip on spreading my thumb and index finger wide apart when I wrap will help.
I'm a little scared of the extra padding, because my therapist put extra on my arm inside the elbow and that caused severe pain where the extra padding was.
Suzybelle, exactly how would you use the micropore tape?
Binney, everything you said made sense! Right now, I don't normally wrap my fingers, but I do get swelling on the back of my hand and the extra padding I put there has never hurt me.

Girl, you can lick this = no worries! You have several experts on this thread (not me)!!!
I just take the tape and tear or cut a little piece to go over the pressure point that is giving me trouble, and then put my gauntlet back on. I was having some issues with the elbow crease when I wear my compression sleeve, but some prosthetic lotion fixed that.
These are all tips I got from this forum. I love my LE therapist, but these women on this forum are my 'experts'. So far, every problem I've had, they've fixed.

I don't think those are tendons: I tried to get an image of thumb tendons, and all I could get was a link to a site about DeQuervain's tendonitis, which shows where the thumb tendons are

IMO, those are inflammed skin folds, and should respond to taking the pressure off of them, and no chafing. I can't do Suzy's trick of tape, as I'm allergic to adhesive, but I'd recommend padding.

Yep, that's how mine looks when it's acting up. Never even thought to wonder what they were
, just tried to get them to stop hurting! For me, the looser wrap and more flexibility of the thumb and fingers, and some soft padding, eases it. We do ask a lot of that joint area with both wrapping and garments.
As you wrap with the short stretch bandages across that area, are you folding the bandage down at that point so it isn't pressing into the joint? I can't leave the bandage flat (unfolded) there or I get that in the web. It's a bit of a trick to fold the bandage there and make sure the fold doesn't continue past that point, but with a bit of patience (
) I can make it work. (When my therapist does it she makes it look so stupid simple!)
Also, for some reason it's hard to keep the short-stretch from rubbing directly on my skin through there, so that's another reason why the extra bit of Artiflex helps there.
Just curious now -- do the rest of you fold your bandages as they cross the web space or not? I'd be interested to know if everybody does that or it's just me. <sigh!>

Binney, due to tons of chafing, I use a 4 cm bandage on the hand, and when I go over it with the 6 cm, I definitely take a fold through the web space--so does my LE therapist.
Also, because that area is chafe prone for me, I have a soft piece of foam that I put in that space: this is really scary because I'm blanking on wrapping after my 4 week hiatus in the cast--but it's coming back to me--I put it in there after I wrap the Rosidal soft and before I start the short stretch bandages--and stil sometimes it chafes.
Man, I think I'll need a remedial wrapping session when this cast comes off....

Okay..... good news! I have successfully wrapped for 24 hours without pain thanks to you guys!
I folded some padding (like batting) over the web space, then I spreaD MY THUMB AND INDEX FINGER WHILE WRAPPING LOOSELY. [ First stockinette, then the extra padding, then rosidal, then short stretch.] Darn cap lock... it's hard to type while wrapped. lol Anyway, I also didn't wrap around the web space as many times as usual. I do fold the short stretch bandages while covering.
So, when I unwrapped the webspace was a little fat, but not much and my hand was pain free! Thank you guys so so so much!
By the way, I don't know what those things are, but they are definitely not skin folds.... it feels like fat threads pulled taut... like a guitar string.
I'm still waiting for the Reid sleeve, which is late.
Will keep you guys posted about that and the CirCaid!

Would using a dorsal pad help with this............maybe just take some of the pressure off the web area?
Binney, I'm trying to picture how you are folding your bandage down so it isn't pressing into the joint? A 6mm bandage fits right across my palm and just slopes up a bit into the thumb. Is that where you fold? Maybe my hand is longer there?
