TCH: ABRAXANE or Taxotere

Hi dear all. I am a Chinese student now studying in Japan. My mom was diagnosed with IDC in China on Sep. and later received mastectomy. Her IHC result is ER-,PR- and Her2+++(FISH:+). We are now discussing the chemotherapy with her onc.

At first her onc suggested AC-TH, which is most widely-used in China. But later we turned to TCH because it has less cardiac toxicity and almost equal efficacy.However, they don't have much experience in TCH in their hospital (I assume they never used it before). And her doc is also worrying about leukopenia and neutropenia along with TCH because my mom has the relatively low WBC count (under 3*109/L).

Another problem is that she is allergic to many drugs. I know Taxotere will bring many allergic reactions because of its solvent. In search of a non-solvent substitute of Taxotere, I found ABRAXANE. But it is protein-bound Taxol, not Taxotere. So can anyone tell me if you have used ABRAXANE in TCH? Or do you have any suggestions on my mom's chemo? Thank you in advance.

    I am doing the TCH. My doctor has given me lots of drugs to counter act some of the side effects. Neulasta is a shot I get the day after chemo to boost my white count. When the give me chemo I they also give me Benadryl. I'm also on Dexamethasone the night before, morning of and night of chemo.I take Aprepitant (Called Emend) day of and 2 days after chemo. I also take Cetirizine (called Zyrtec here in the US) and pantoprazole (called protonix in the US) all the time to counter some of the side effects.

    There are other drugs I have been given for side effects but I haven't needed them yet.

    Not everyone gets neutropenia or leukopenia. Granted the Neulasta shot takes care of that for me. I have not had leukopenia so far. I just had my 3rd treatment of 6 on Tuesday.

    Sorry but I don't have any knowledge of Abraxane.

    I had Abraxane rather than Taxotere..or so I thought. I was not aware that it was taxol rather than taxotere. However, that would not have swayed me away from Abraxane at all. The Abraxane is typically not given to early stagers and is not always easy to get insurance approval for stage 1. However, I have hand/nerve issues as well as carpal tunnel and also needed to work through chemo. My Onc. was able to push it through for those reasons. His thought was that although any chemo would likely aggravate my hands, Abraxane would be the kindest since it does not require the pre meds which in of themselves can worsen my hand issues.

    With that said, I am so grateful to have been able to get Abraxane. There was the simple benefit of shorter treatment time (under 3 hours for each tx) and the bigger benefit of no neulasta, decatron etc. I did experience progressively worsening nausea and just wish I had asked for Emend sooner because that was a lifesaver! But truthfully, I think that was caused more by the Carboplatin. I say this because my WBC was very low for my last tx and because I was scheduled for surgery a few weeks after, my Onc. omitted Carbo for the last tx. I felt infinitely better without it!

    I was also put on a 3 week on, one week off treatment schedule as opposed to the typical once every 3 weeks. The reasoning was that I might do better getting smaller doses at a time while continuing to work. Though I still did have to take some days, I never went a full week feeling too sick to work. While schedule did serve it's purpose, I never felt good for an entire week either unless it was my week off!

    I hope my experience gives you a little more insight. The bottom line is I was thrilled to be able to get the Abraxane!

    Thank you very much, lago and REKoz. We finally decided to use Taxotere. If it is not well tolerated, we might switch to Abraxane.
