Mastectomy w/positive margins

I'm 42.

I don't know if this is the right place, but here it goes...

Three weeks ago, I had a skin sparing mastectomy (stats on bottom) with immediate reconstruction (actually an expander was placed/ augmentation on left side).

Everything went well (besides a collapsed lung when they accidentally poked it while inserting the port)...but today my surgeon told me that there was a positive margin on the skin side of the lump.  It was right up against the skin, less than 1mm away.

He said my cancer is aggressive, and that he can go in and remove the skin that was above my lump "if" he can find it.  It would require taking out the expander and not be as nice to look at when finished.  (No big deal.)  He also said surgery for positive margins after a mastectomy is controversial because rads is a viable option instead.

I told him finding the spot of skin wouldn't be a problem because all my bruises are gone except the place that was right above my lump (which was pulled down a little when they sewed my nipple area back up.)  It is still bruised, but appears to be fading.  He looked at it and didn't seem concerned.  Yet.

I didn't let them take my axilla.  So, because my 4 sentinels were positive, I am having radiation (After chemo) on my axilla and my surgeon said we can also zap that skin area above where the lump was with rads, and let the chemo and rads take care of any potential skin issues.

My question.  Has anyone had mets to their breast tissue from IDC after a mastectomy?  If so, how long after surgery did you see it?  What did it look like? 

My PS told me not to worry, that it would appear on the skin and be caught early if there is a local recurrence.  But damn, recurrence makes me want to gag.

    I had a lumpectomy not a mastectomy and I had a positive margin over the skin as well. My path report said I had 30mm extensive component of IDC and DCIS present in the margin.

    My BS and Rad Onc thought the Rads would take care of the positive margin so I didn't have any further surgery, my anterior margin was only 1mm, but no further surgery was recommended as that's the chest wall margin and he couldn't take any more tissue.

    I'm coming up to 2 yrs out and no problems so far.

    Thank you Lou Lou!  That makes me feel soooo much better....my drs also think chemo and rads will take care of any left over messiness.....
