Plain language treatment plan?

Hi all,

My doctors just talked me through treatment options and their recommended approach.  It was frustrating.  I wanted a 'one page' version that would give me the basics at this very stressful time.  I'm working with my treatment center to make one.  

Did your team give you a document you thought was great?  Or is there a one pager out there on the web that you'd recommend?
