Just Diagnosed w/ DCIS- Have Qs
Hi everyone. I'm 60 years old, very active and fit, and was just diagnosed on Friday with DCIS after a stereotactic biopsy. I had tightly grouped calcifications(5 mm) in my left breast found on a mammogram. I am having a MRI this Friday and seeing a surgeon in the afternoon. I've been reading a lot to learn as much as possible before Friday. I would love some input into what questions I should be asking the surgeon at this stage. Thanks in advance for your help.

Hello, DMS.
I hope that you are doing well and that I'm not too late to make a few suggestions. Ask for copies of your of all of your test results. I think that most of us like to have our own copies for our own files. Hopefully, the surgeon will have results from your MRI, so also ask for those results. I would want confirmation from the MRI that the DCIS is confined to the area shown on the mammogram. If the surgeon recommends one specific type of surgery, I'd also like to know their reasoning for recommending that particular surgery. Also, what do they think will be the next type of treatment. Do they think radiation or Tamoxifen would be beneficial after surgery?
Goodness. I may have tossed out alot of suggestions, but these were some things that I thought about. Consider writing your questions and the answers from the doctor. Or, if possible, bring someone with you to serve as a second set of ears.
Best wishes. Let us know how the appointment goes. We can probably help you with some questions and opinions, too.

DMS,
Ditto on wyldbluemusic-get a copy of all your reports. It helps to have all the terminology when reearching info. Do you know what grade your DCIS is? Last night I listened to a very good vido on understanding DCIS on the Susan Komen website. It's 90 minutes long but the first 30 mins has the most information.

Best wishes. You have come t the best website. I have found Breastcancer.org to contain the most info.

Hi DMS
Yes, you are in a good place. I learned so much here. I agree--get your reports and have questions ready.
Initially it was recommended that I have lumpectomy and then wait to decide on radiation and/or medication. I appreciated that my BS was not pushing me in any direction until after the lumpectomy and the pathology report. There is no rush to treatment and I think it is important that patients stay calm and don't jump to a decision.
One question I would ask is if after lumpectomy the lesion is smaller or larger than thought what would recommendations be in either case? The pathology report from a lumpectomy surgery would tell you exactly the size of the lesion and the margins. This is important for informed decision.
If radiation is a possibility you may want to ask for an appointment with a radiation oncologist to find out options for your particular situation.
I did this and I also consulted with a plastic surgeon just to know what options were available to me.
I did not have an MRI--my radiology path report stated that DCIS is "underestimated on MRI". I wonder what other person's experiences are with MRI. It was not recommended for me. I am being treated at Mayo.
I have been told that the treatments women get for DCIS depend on their particular situation, personal wishes and the area of the country they live.

Thanks everyone for your input. My MRI was clear and I had a wide area excision on Monday and am feeling pretty good. Today, I just got the pathology report from my surgeon -- low grade and wide margins. Yeah!! The original biopsy came back as DCIS moderate to high grade. Will be seeing my surgeon next week and then a radiation and medical oncologist.

Just want to say I am almost eight years out from DCIS diagnosis and treatment. Single 4cm. tumor. Lumpectomy, rads, and five years of tamoxifen. I am fine today. None the worse for wear. Wish the same for you and everyone.

DMS - congrats on your results! If your grade is low enough and your margins are wide enough, you might be able to go without radiation. I'd definitely ask, and maybe get a second opinion on that as well as the pathology slides.
Radiation wasn't so bad (at least for me), but it would've been nice if I could've skipped it.
You might also want to get opinions from 2 different medical oncologists when it's time to have the hormone pill discussion.
(I believe in talking to more than one Dr before I make serious decisions.)

Thanks ladies. Your input and support are wonderful. I'm so glad I found this forum. I walked in the Race for the Cure in June in support of a friend with Stage IV metastatic breast cancer, never thinking in just two weeks, I would be a member of the club.
I need to find out why initial biopsy showed medium to high grade (stereotactic needle) where they got most of it and the latest. (Different readings or outer edge being lower grade?)

Congrat on your results! I'm having lumpectomy w/SNB on Tuesday. Anxiety has set it again, as I had my pre-testing this morning. This has been quite the emotional ride! I hope and pray for clear margins. My course going forward will include radiation and then tamoxifen.

Linda603- My thoughts are with you. Hope everything goes ok for you.
-
Linda, I'm having the same surgery on Friday...a couple of days after you. I have so much anxiety already that I can't even imagine what it will be like towards the end of next week.
-
Dear Janet: I hope you are recovering comfortably. I'm still having moments of anxiety, as I await my pathology report. I will know late today or tomorrow. I keep thinking positive and family and friends have been amazing. I'm back to work, driving, etc., and feel great. I wish you a speedy recovery and clear margins!
