Outcome of ADH Slides retested? Still ADH or DCIS?

Helllooooo,

I have been diagnosed with ADH and although it was at a reputable large Hospital with a Breast Care Center....I am thinking of having my slides re-tested as I have heard that there can be a very fine line between ADH and DCIS, and that treatment options would be different based on the findings.

I was wondering how many of you diagnosed with ADH have had their slides re-tested and what the outcome of the results were for you?

    This older paper  (1999) gives some idea how often pathologists disagree about several breast conditions. http://www.ncbi.nlm.nih.gov/pubmed/10071228  Since DCIS and ADH are treated differently, if you have any doubts, I think it is in your best interests to get a 2nd opinion.

    Hello, sorry for being late with a reply.  I have a current diagnosis of ADH as well.

    Definitely have the slides re-tested.  Apparently there is a quantity of cell difference between DCIS and ADH, not a quality difference.  I saw a stat that I can't put my hands on at the moment, but 1 in 4 pathologists will see a different conclusion in the slides.  And, there are some doctors who specialize in reading the slides.  I'll see if I can find the cite others have recommended from here.

    I was diagnosed last year with DCIS.  I shopped 3 different doctors and not a soul mentioned any difficulties in just going in and taking out the DCIS.  The lumpectomy was late Aug 2009.  My follow up mammogram in June 2010 showed more calcifications.  Only then did I call and receive the official pathology report from Aug. 2009 lumpectomy.  There was ADH all over my slides and the treating surgeon NEVER told me.  After the calcifications showed up, I wanted a different doctor any way (yes, a 4th) but he's the head of breast surgery and he's ordered a full re-evaluation of everything from last year, including the 71 slides from the lumpectomy with all the ADH in it.

    An MRI from July 2 showed "fair amount of disease from nipple throughout breast", but I have had to wait til now to talk to the doctor about all the results of everything.  I finally see him tomorrow.

    I hope you have found someone to relook at your slides, and all has gone well.

    My experience was similar to Tara's - only i knew what the reports said, but i didn't want to think about it. The original finding was: suspicious calcs in 2 areas (or 1 large area), other scattered calcs as well. The double needle/core biopsy said ADH and DCIS. The excisional biopsy, again double, showed IDC and DCIS at one site, DCIS at the other.

    The 2nd opinion pointed out that there were more calcs in other areas, and recommended mx. I demurred, and they said - at least get an MRI to see what else is going on. The MRI was clear, so i accepted the double biopsy with its nice clear margins as a lumpectomy, and did rads.

    The next year, my mammo was digital instead of film, and showed more detail on those other calcs: suspicious now, and 7 cm, from nipple to near chest wall. The core biopsy showed ADH and typical hyperplasia, nothing more. The 2nd opinion on the slides was the same, and my onc thought i should just watch-and-wait, but i went to Mayo and they said mx. Which i did.

    So i would say: for the 2nd opinion, get the mammo re-evaluated as well, in case there is more going on in the breast. And have a digital if the original was film.

    On MRIs -- my onc told me that they are best for seeing increased angiogenesis (a mark of invasiveness), while mammos are best for calcs. Which is why my MRI was clear.

    I was diagnoised with ADH ?CIS from my stereotactic biopsy.  Tomorrow having a excicional/lumpectomy.  I still scared this is all new to me.  My mother had ovarian cancer.

    ashland6336 - read all you can - the basics and beginning cancer stuff, so you'll have some terms that are familiar to you when they start telling you the results of the lumpectomy.  It is very scary until you know exactly what's going on.

    Hang in there and even if you read scary stuff, press on and understand what all the possibilities are.  Hopefully, you'll be very relieved at the results.

    I got a second opinion and got another diagnosis.  I was diagnosed with intermediate DCIS by a well respected hospital in NYC but went to another major hospital in NYC and was diagnosed with ADH instead.  The second hospital is a major cancer hospital, knew about the first diagnosis, and still gave me a diagnosis of ADH after reading the same exact slides that the first hospital read.  My surgeon said that it is controversial but wants to do surgical biopsies before sending out the slides to other hospitals.  I just had surgery as if I had DCIS (wider margin than ADH) but I'm ok with that.  I"m waiting for more pathology results before getting more opinions.

    I think it's smart to get another opinion particularly they are supposely so difficult to tell apart.

    Hi!
    I was diagnosed on Friday with ADH and I am thinking I should go for a second opinion.  I am only 31 so I would rather be agressive than 'wait and see'.
