DIEP flap belly shape
Hi - this question is for women who have had the DIEP, which I did, a little over a year ago. My problem is my belly shape which causes me to be unable to wear anything high rise as it just rolls off, so I wear low rise pants now. I'm having a heck of a time finding comfortable jeans that are able to stay up but aren't too tight in the legs. I know I can stand to lose some weight but haven't had luck with that lately. Anyone else dealing with this? Ideas?

Hi jillisuecat: My stomach is shaped funny also.. I think I needed about 10 more lbs of fat taken out of my middle section, but I know they can't do that because of insurances... I think after the fat was taken for the boobs and everything else resettled, it all went to the waist section..

I'm printing both of your postings and taking them to my plastic surgeon and going to ask him am I going to have this problem too ??? I had a diep flap mid feb but am dealing with edema right now. I still need reconstruction, a lift of my remaining breast and the pointy sides lypo suctioned off at each end of the tummy tuck. Dr calls them his spare parts for repairs. The one thing I do not like is the "extreme" tightness of the abdomen. It's compressing my dyaphram and messing with my breathing. Is anyone else having this problem ?

jilllisuecat ~ Not sure I understand your question. Are you saying there's a really significant difference in the size of your tummy above and below your incision? If so, I would think your PS could lipo the upper area to remove some of the excess and make the transition smoother. Have you had your Stage II yet? Sometimes the lipo'ing is done then, if needed. Hi dib...not sure if your question is for me or not. No I have not gone beyond just the diep flap yet. I am bloated and retaining 15 to 20 lbs of fluid right now. A good 4 lbs is in my right arm. It was under control until about 3 wks after the sugery then it all went to "hell'o there". Not sure if swearing is allowed in here or not. My GP put me on 3 days of diaretics and potassium today because the bloating is even visible in my face. She agreed I have half my wrinkles, think I'd rather have my wrinkles. Working on getting this under control so he can do the lift on my left breast. Now I've been reading other posts about strange tummy shapes and thick waists after diep flaps...does it ever end ??? Hey how come no one uses the live chat ?

Hi - I had a TRAM flap. At first my belly was so swollen I could not wear any of my pants or skirts. I am 14 weeks post op and can wear some items. I hear what you say about the low rise pants. I bought some for a March vacation, they actually sit mid waist. My belly is shaped quite oddly, straight up and down. I too could benfit from some lipo reshaping (my insurance will not pay for that part of it, don't know how much it will be but going to do it eventually). My belly, perhaps entire body has been intermittently retaining fluid. It self resolves so for now I'm staying away from diuretics.

I dont get the whole insurance thing, my dr has always said Lipo is part of the procedure for stage 2 and has never mentioned my insurance not paying. Are some Drs just nicer than others LOL??

My original surgeon was willing to lipo around dogears at my hips after DIEP, but anything else would have been extra. I had to head to NOLA after he would not reoperate to correct a complication, and Dr. DellaCroce has done a lot of lipo for me at no extra charge. The NOLA docs are known for their generous resculpting to give us back our curves, because DIEP pulls everything downward and can give you a boxier shape with no waist. My pants fit differently, but I was able to switch to another brand which fits much better.

I'm almost 12 weeks out from my bilat DIEP surgery. All the post-op swelling has gone down, but I have a "belly" still. Not flabby, very firm, but it definitely is raised more than my hip bones. I can tell there is little fat under the skin because when I flex and tighten my abdomen muscles, I can feel them right there.
I suspect that only dieting will take care of the bulge, assuming it is not something else like a hernia :-( I was told once about the different kinds of fat we have in our bodies. The yellow fat is the kind we have as padding just under our skin. That is the kind that is removed through liposuction and short-term dieting. Then there is brown fat which is the padding we have deeper in our bodies around our organs. This is harder to remove, and truthfully, you don't want to remove a lot as it helps stabilize the organs and keep your body temperature constant.
I think all my intestines are closer to the surface due to extra brown fat in my abdomen and they're part of the mound that I have between my hips. Lipo won't touch that kind of fat (too deep), so that's why I say only dieting willhave any affect on it.

My doctor won't do any tummy lipo unless I pay, says it isn't included in the insurance.

Hi Pheosky,
I had my tram flap in Jan and had alot of trouble with my breathing due to my abdomen being so tight. It took over two months for that to get better. I have been exercising and trying to lose the extra weight I gained for the tram flap, this has helped with the tightness and my breathing.I had a lift on my healthy breast ten years ago when I had cancer for the first time. Be prepared for discomfort. I think my lift hurt more than anything that was ever done to the reconstructed side. Hang in there, the tightness will get better. good luck

I can relate to your pants rolling down. I had my Diep in January and my tummy looks a little bloated. Ya I need to lose some weight which I am having a hard time at. Today I went to my PS for a consultation of my stage 2 of my diep and they told me that they will try to tighten up my tummy but he might insert into my abdomen something like a girdle that stays in my belly and pushes my belly in. He told me that if he does do that I might have to go home with a drain. Not looking forward to that. That scar that we have from the diep does it ever fade or go away. I still can not believe how long my scar is.

I didn't realize how my lower tummy roll was holding up my pants until I had a diep. Now without the lower roll, my pants do tend to fall down. I don't have much of a rear or even hips, so I've got to keep an eye on things! Just had Stage 2 this week, which should hopefully give me a bit more of a waist through lipo, but it remains to be seen, since I'm still swollen.

WOW I thought I was the only one with this issue. My upper belly is larger than my lower which makes things fall down easier. It is very odd. My PS also would not do any lipo unless I paid out of pocket (which would have been around $6,000.

Sigh... still, I'd rather have this issue than the belly fat they took, so most of the time I don't worry about it. I don't want more surgery anyway! I've never even gone back for a stage 2 - I don't want to have to heal again.

My bulge seems to get larger and then smaller depending on a number of things like what I wearing (how constricting the waist is); How long I'm sitting or leaning forward; what kind of meal I've just eaten; If I've laid or slept on my stomach (that makes it flatter). I'll be seeing my PS this Friday for my nips, so I plan to ask him about it then.

The extra puffiness from sitting, etc, was explained to me as fluid build up, which gets blocked from it's natural drainage by our wide scar. My lymphedema therapist massages a line from my belly button over to my hips to assist that drainage. Both she and the PS said that over time our bodies recreate the drainage over/through the scar tissue, but until then (I have no idea how long that takes) massaging can be helpful. Do you have swelling anywhere else? If you do not have lymphedema, you can still make a consult appt with one to discuss this. The certified ones do know about this in the connection of extra swelling. The general fullness is part of the original procedure, though, which is unfortunate. If there is an option to do some lipo at Stage 2, that could be helpful. I figure weight loss is in my future either way.

It's nice to know I'm not alone, can't keep my pants up either. It's been almost 8 months and I've grown more than a little tired of hiking up my drawers every other step. I think my problem is fluid buildup, the area around my belly button jiggles a lot differently than the 'fat parts' jiggle..lol. I'm going to give the lymphedema massage a try, I hadn't thought of doing it on my belly. That's what I love about this site, there's always something to be learned here. There was a really informative and totally funny thread about big pelvic mounds after DIEP's - anyone else have that problem? I'm not even going to mention the 20# weight gain....
Good luck with your nips on Friday Nancy. I've been waiting for Stage 2 for 2 months now and don't even have a surgery date yet (sigh). Hopefully some lypo will flatten out this belly, or at least carve out some semblance of a waistline or I'm going to have to start wearing suspenders....

To all of you that have the "Pants Rolling Down" problem, not to mention the pain of sitting down in most pants!. I had a DIEP flap done on February 10, 2010. I have been tender in the tummy incision area (didn't realize that the incision would actually go from hip-to-hip). I only wear "Yoga Pants" these days. They're so comfortable and yes, they do sometimes roll down but they are a lot more comfortable than jeans and other "Regular Pants". Easy to order on-line and not very expensive.

Nips are done! It was easy-peasy. I almost slept through it. They're covered up until Sunday when I change the dressings for the first time.

My PS and I discussed lipo. It would definitely be an "extra" not covered by the DIEP fee. I don't have a lot to do, just smooth out a small bulge on one side of my tummy scar, and trim some little dog ears. I could almost live with them, and I might just decide to do that.
I have lost some weight since my surgery, and plan to stay on my diet with the goal of losing 25 pounds (I've already lost 5-7). When I reach that goal, I will see how things have shaped up. I might end up with more belly skin to remove.

I had my DIEP 13 months ago. Stage 2 is not scheduled yet but PS would remove dog ears from my hips, lipo one small area on my tummy and lipo love-handles. He likes to do stage 2 18-24 months after stage 1 so everything settle down.

I am ten days post op from my bi lateral diep. I have a question about the purpose of the binder. Is it to support the incision or to hold my belly in so it can reshape? How long did y'all wait before scheduling stage two? Also, am i posting in the right spot?

My DIEP area is a bit lumpy.
My surgeon only does 1 surgery - no Stage 2.
Should have researched this before I had my surgery...sigh.

pinksea, the binder is to keep swelling to a minimum. Stage II is typically at least three months after stage I, as long as you have healed well.
Ang7, there were huge differences between my first surgeon and Drs. DellaCroce and Sullivan. You could have stage II in NOLA---insurance does cover it and these doctors are known for the sculpting and shaping they do to give us a great waistline after DIEP. I have weight still to lose, but have really nice definition to my waist now and no problem keeping my pants up. Dr. D has done lipo to my upper abdomen a couple of times and it is much better. However, any weight I gain goes right to that area thanks to menopause.

Thanks Sandy. I have been wearing the binder 24/7 and still have the two tummy drains in. I am looking forward to getting those out next week. It sounds like swelling remains an issue until around week four. I'm healing well and looking forward to stage two. Do you think there are advantages to waiting longer than three months for stage two?
