I did not have this procedure in the end, b/c I didn't end up having a mastectomy.
I discussed skin sparing/nipple sparing as an option with breast surgeons at both Columbia Medical Center and Sloan Kettering in NYC and with Dr. Joshua Levine (plastic surgeon in private practice, not really associated with a hospital), also in NYC.
I also saw a surgeon at St. Luke's Roosevelt who strongly believes saving the nipple is not worth the risk and will not do these procedures.
I'm happy to provide more info if anyone wants to PM me.

Many of the breast surgeons do the procedure if you are a candidate. Dr. Sachini is one of the pioneers who started the procedure over in Italy. http://www.mskcc.org/prg/prg/bios/629.cfm
Dr. Montgomery did my procedure at MSKCC she is now at Montefiore http://www.montefiore.org/whoweare/stories/Lesliemontgomery/ She was not a big fan of the procedure, but she did a great job,
