DIEP Stage 2
I had Stage 2 of my DIEP yesterday. No more dog ears and he did some lipo in a few places to even things out. I had a feeling that lipo would lead to soreness, but that is an understatment. No wonder my PS said to feel free to use his name in vain. I sailed through Stage 1 with very little trouble, but today I feel like someone worked me over with a baseball bat. I've used more pain pills today than I did during the first couple of weeks after I got home from Stage 1. I've always wanted to do a little lipo here and there on my thighs, but I think this experience will cure me of any such notion! I am one big ouch.

really? good to know. I am having stage two -lipo and fat grafting in a little over a week from now.
would you say that you can walk? are you retaining fluid? are you wearing acompression garment?

I had Stage 2 with lipo/fat grafting on Feb 3. I was very concerned beforehand about the pain of lipo. My PS said that the type of lipo he does is not like cosmetic lipo where they aggressively use large canulas, etc. He said he takes very thin slivers of fat and layers them. He said to expect lots of bruising but little pain. I was skeptical, but he was right! Pretty nasty bruising with minimal pain. I've had more pain from banging my shin on the dishwasher door! I didn't have to wear any compression garments. It's been 16 days and the bruising is barely visible now.
Just thought I'd relate my experience. You may have had more extensive lipo than I did or maybe our PS's used different methods. I'm sorry you're having so much pain! Thank goodness your Stage 2 is done. Here's to healing quickly!

Let me preface this by saying that I view myself as a total wuss when it comes to pain, but I certainly did far better after my DIEP than many of the ladies who posted their experiences. I think that is why I am so surprised that the lipo affected me this way.
I can walk and have certainly been up and down, referring to both bed and stairs. Despite the discomfort, I have stayed busy with primarily sedentary activities. Today I decided to be a hair more active and did some very light housework and sewing. I am lovely shades of black, blue and purple and am quite sore, but it is not disabling - just painful. He also extended my belly incision about an inch on each side as part of the effort to get rid of my "points" (dog ears) and those areas are pretty tender. The more I move, the less the pain, but not unexpectedly I am so tired that I am dragging my tail behind me. After all, this was my 4th surgery (2 lumpectomies before the MX/DIEP) in 6 months. I'm going to be glad to be done after the next procedure.
Getting up from bed or a chair after long periods of being still is the absolute worst. Everything is so tight and painful, but loosens up with movement. I suspect that I am not going to feel like running a race for at least another couple of weeks, but anticipate that it will get a little better each day.
Hopefully, you have a higher pain tolerance and will have a better experience. Good luck.

TOB, I'm glad you started this thread. I'm scheduled for Stage 2 in April, currently being 4.5 weeks out from bmx/diep. I was told that Stage 2 leaves one feeling soreness like they've really overdone it with exercise. I got so much good info regarding Stage 1 that I felt very prepared and was able to be fine with that. There is less info on Stage 2.

Hi folks, Stage 2 is really a breeze if you don't have lipo. My PS will do lipo,but the hospital charges if there is anything extra. I was kind of sad about that. Other wise, I was not on any pain meds and I was driving in a couple days.
My surgery was about 3 1/2 hours and I stayed over night because I have troubles coming out of anesthesia. If not for that I would have gone home that evening. I also wore a compression bra 24/7 for 1 month.

It was not my intention to scare anybody, but I felt that honest information would be helpful to ladies who came after me. I looked and didn't find very many ladies who said anything abiut Stage 2 other than it was much easier than Stage 1, but I'm not so sure I would agree with that. My surgeries have been at one of the leading teaching institutions in the country and I have the utmost of confidence in my PS. He warned me that I would be sore, but it is clear that he has never had lipo!
I am now 5 days post Stage 2 and the worst is over, I think. I was in real pain the first few days, but now feel, as another lady reported, as if I really, really overdid an exercise routine (and was hit a few times with a baseball bat.) I am still very sore in some spots and very black and blue, but getting along well on Motrin rather than narcotics. Wearing most clothes is not fun so I am living in sweatpants (something I never owned prior to my MX) and camisoles with shelf bras. I tried a real bra and I am just too sore in a couple of spots thanks to lipo.
Frankly, I think the hardest part of this whole thing is the sleep disturbance and postop fatigue that I've been fighting since my MX/DIEP in mid-November. They so disrupted my already fragile sleeping routine during 2 1/2 days in ICU that I still haven't gotten it straightened out. I wake up many times each night due to pain, discomfort or for no identifiable reason. Fatigue after any major surgery is to be expected and it can take 6-12 months to get back to your preop energy level. This was my fourth surgical procedure in 6 months so I expect some fatigue, but knowing that doesn't make it any easier to live through it.

Thanks again for starting this thread. I hope you are better! I had my stage 2 yesterday and am quite sore. No lipo but my breasts and left underarm area is painful & achy. I'm taking vicodin es every 4 hours, which isn't helping as much as I expected! All in all it's tolerable. I'm trying to sleep as much as I can these first few days ... sleep is very healing for me.

Well, guess I better give my experience. My stage I was extremely hard to recover. Stage 2, I had on Thurs. and wasn't bad at all. Used no pain pills and was back at work Monday. I guess everyone is different plus some may have had more lipo than others. That's probably the biggest key.

I appreciate the details you are sharing regarding Stage 2! Maybe the difference is in the amount of lipo, as mikita suggests.

Just saw my PS Tuesday for my first and only post-lipo check. He tells me that he did not do all that much lipo and used the smaller cannula and most advanced lipo technique, so we agreed that I must be a wuss with a low pain threshold. (I already knew that.) I am still pretty sore in a couple of places, but it is getting better. Everything looks very good and I will go back in June for the nipple procedure. The hardest part is no exercise or weights for at least 4 weeks. I miss my vigorous workouts and will need to start back slowly letting any residual discomfort guide how much I do. For now, exercise is limited to daily 2 mile walks with my husband and our two big dogs as we continue to enjoy an amazingly mild winter in the Pacific Northwest.
Everybody is different, but I still stand by my original assertion that Stage 1 was actually easier for me than Stage 2, especially if you compare the number of pain pills I needed after coming home from each procedure. I'm probably one of a tiny minority to say that, but here's wishing all the future DIEP women both an easy Stage 1 and Stage 2. I am so looking forward to being done with this and getting back to my life without any further interruptions.

I had Stage 2 DIEP revisions on Jan 25, and it was a breeze. But, I didn't have lipo. So, that must be the key. I would have to say too, that I thought Stage 2 was easier than Stage 1, although Stage 1 recovery wasn't as bad as I thought an 11 hour op would be.
Glad you're feeling better now! My PS let me do cardio at the gym after the first week and after a month I was back to strength training. Actually, I'm still trying to get "back" into it... I lost my rhythm with all that time off, and I'm not all that good about exercising. So, it's back to square one on that.
Know what you mean about looking forward to getting your life back.... Me too!

Good morning Ladies,
After a long 5 weeks, I am finally meeting with my PS tomorrow to consult about scheduling Stage 2 DIEP. I cannot wait. I haven't been able to really wear a "real" bra since my surgery on Dec. 11 because my foob is slighting larger and rounder than my real one and it's just uncomfortable. I am so anxious to get this next step going. Is it really typical for PS now to do lipo? I do have a slight dog ear on one side and would love to get some "shaping" done on both sides. He never mentioned lipo during our other visits so I am not sure what his protocol is. I don't want to sound ungrateful or anything because he's done a fabulous job so far. It sounds like it is becoming pretty standard to have lipo done at Stage 2 though. Can you guys offer any suggestions on things to discuss when I am there tomorrow. 24 hours and counting .... Thanks so much.

Besides "dog ears," is there any other reason a ps would do lipo at stage II in terms of breast shaping?

Good morning! I saw my ps this week and he was very pleased with how things are progressing and is ready to get me scheduled for stage 2. Although alot of surgeons do the nipple at this time, my guy doesn't like to do it then because I am having a slight reduction of both breasts along with lifts. He wants to do that first and when that is healed then he'll do the nipples. His reasoning is that if he does the new nipple now, there is a chance the healing could put with me with one nipple pointing up and one pointing down. Not only is he cute, he's a perfectionist too! I am fine with that. He will fix the dog ear on one side and when I asked about lipo, he said typically insurance won't pay for it. He didn't seem to think I needed lipo even though I have a little area that I would love to get rid of. When I told him that I had lost about 50 lbs several years ago and could never seem to get the weight off in my belly and sides, he smiled and said he'll see what he can do. Oh, bless his heart, I love him!

You may want to ask this PS directly if he will help you take care of your tummy, so you know. If you had a DIEP, there is a possibility that he can work out lipo for you while doing "scar revision", or something like that on your tummy. Don't hesitate to be very direct about this, since it is important to you.

My next revision surgery will be in two weeks. My PS did say that he was going to "take care" of the extra skin/tissue (let's face it -- it's FAT) under my arms. Will he be doing lipo for this?
Also, what are dog ears? Who knows, I might have those, too!

"Dog ears" or "angel wings" are little poofy skin projections above the hips on the sides at the very end of a long incision line which will need to be smoothed out later. Especially with a DIEP, there are very often dog ears on the sides. I have one which is nearly flat, and the other is obnoxiously sticking out with great gusto. I'm not sure how they "fix" these during stage 2, but it seems like they could easily just cut them off and stitch now that everything else is unswollen and healed. We'll see. Stage 2 for me is Apr 21.
As I understand it, underarms are done by lipo most often, unless you have a lot of very loose skin. How does this impact lymph flow? Does anyone know?

oh, well, I DO have dog ears, too -- just didn't know what they were called. He said he is taking care of that, too, during next surgery. Those are what he will use for the nipples/aerola. Will have to ask about the lymph flow -- that is a good question.

Stage 2 lymphatic flow is my biggest concern. Heck, I'd have them lipo practically my whole body if it didn't interfere with lymphatic drainage!

I had bilateral mastectomy with immediate DIEP reconstruction in August 2009, then my Stage 2 revision in November 2009. When the Stage 2 revision was done, my surgeon revised my abdominal scar, took care of the dog ears, nipple reconstruction, breast lift and did some contouring (lipo). He told me that it would be like a bad paper cut. I did not really have much pain from it at all. I had some discomfort which included a feeling like I worked out at the gym after not having been there for a while. I did not have any bruising or discoloration to speak of.
As far as lymphatic flow, I have continued to do some skin brushing to try and help things move along just preventatively.

How long to you remember the soreness lasting? Those are my 2 questions - lymphatic flow and length of soreness.

I am scheduled for stage 2 on Thursday and am so excited about repairs plus revisions and finally symmetry! My PS will be lifting and reducing along with some dog ear repairs and lipo for contouring of my hips. I lost about 60 lbs 7 years ago and just could never get the extra flab off from my hips. I have a nice flat belly now and am looking forward to slimming down those hips a little too. I was told they'll fit me with a surgical bra before I come home but nothing was mentioned about any sort of special compression undies. Should I be out purchasing one or will they fit me with something special in the hospital? Just curious what to expect but I am ready ready ready to wear some cute dresses come this summer! Thanks for any tips you all may have.

hi- I highly recommend "Higher Power Spanx" I ordered two pairs through ebay and now (four weeks later) wished I ordered 3 . You will need to wear them almost 24/7 . they are awesome. I wash them in the washer and dryer and they are starting to wear in some places. They go all the way down to the knee and up to under your breasts.

I am so glad to have found this board, just found out today stage 2 of my DiEP is May 21st and you all have such good information. I am going to see my PS April 15th and have mentioned the dog ears and he said he will fix that. I also asked him about some dimples in the scar where my nipple used to be. He said he is going to watch that and will make sure it is smooth. I feel like my belly button is higher than it used to be, anyone else??

Stage 2 is done! Surgery was yesterday at 10:30 and I was home in my recliner by 6:00. They revised the dog ear, lifted both sides and did some lipo for contouring. I was sent home with a compression band and a surgical bra. I hate the compression band so pulled out my spanx and am just wearing that instead. The surgical bra is mostly comprised of velcro and it was rubbing so much against the incisions around both breasts I ended up putting a cami on underneath the bra so it didn't rub so much. My ps said I need to wear it 24/7 until I see him next Tuesday again. I do have some bruises where the lipo was done and some major swelling around the breasts but holy cow, my girls are perky. Other than still being really tired and sore (which is to be expected) I am good. I am taking all next week off of work. Hopefully that will be ample time for recovery.

Just a quick update, the surgical bra they gave me in the hospital was for the birds!! Even with the cami underneath, I felt like it was doing nothing and was rubbing so much that I had to pull out the bras I used after my mx. They were a little large though so I ended up at Macy's this morning and picked up a Barely There bra. So far, it's comfy and I think I could even sleep in it. I see the PS tomorrow and will ask the question that I am dying to know the answer to -- when can I go braless?? Does anyone really know the deal with why we have to wear bras 24/7 after the lift? I assume it's strictly to reduce swelling, but my PS never even really put this on his list of restrictions. I had to ask him about it.
I starting taking Arnica yesterday (read about it before but completely blanked out about getting some before surgery) and it does seem to be helping with the bruising. I stopped at the post office today and instead of pushing the door open with my arms, I used my butt to push on it forgetting all about the bruises from the lipo. WOW, did that hurt!! Olivia, try and get yourself some Arnica and start taking it today. Google it to find where you can pick it up locally. I got it at Vitamin Shoppe and it was really inexpensive.
All in all, this stage is so much easier than the first. More soreness but nothing that restricts you to a chair or bed like before. I couldn't understand how they said only a week or so out of work, but I can see that by next Monday, I'll be ready to go back to work. Well, maybe not ready but able :-)
So much for a quick update, but nonetheless, an update.
Enjoying the sunshine and birds chirping ... it's a beautiful spring in NC ...
