Post-mastectomy problems with drainage and necrosis...

Just wondered if anyone else has had problems with their wound drainage and/or tissue necrosis after immediate reconstruction? I bled a lot during my operation and had to have a transfusion, and I'm now three weeks out after the mastectomy and STILL wearing a drain out of my back (where the tissue was taken for reconstruction) and it's driving me NUTS. I'm worried all the time that it's going to get infected etc. etc. and I have a rash around the area that feels itchy. Also, my new breast was doing fine until the surgeon noticed a black spot developing, and he said it was quite extensive necrosis and he removed a ball of tissue (almost as big as a golf ball...) which feels like a setback. I'm not sure what it means, other than more scarring, and longer recovery time. They used manuka honey to fill the hole, saying it has amazing healing properties, but it is starting to smell bad! Ugh. Guess I just needed to have a moan - sorry - and wonder if anyone else has had similar problems so I don't feel quite so sorry for myself. I have no right to feel bad because I have no invasive cancer to worry about, but I didn't realise how low I might feel after the whole thing. What kind of experiences have you had?

Hello-
I didn't have drainage issues, but I did have major necrosis along both incisions after bmx. A good two inches of black scabbing (sorry,it isn't pretty!) instead of normal incisions. I believe it was caused by too much skin stretch and overfill from my now fired PS. Regardless, 2 weeks of daily IV Vancomyicin took care of the infection. A few months later, I had an in office debreidment (sp) procedure. I came out with nice, thin and clean incisions.
Keep an eye on that rash and your temp. The big gun antibiotics can save some expanders. Good luck and God Bless

I HAVE DEBATED WEATHER MY EXPERIENCE WILL HELP YOU OR MAKE YOU FEEL WORST.I HOPE BETTER.I ALSO HAD NECROSIS PROBLEMS BIL. BACK TO SURG. THOUGHT WE HAD "DODGED A BULLET" AS MY PS SAID. THAT PROBLEM DID RESOLVE-BUT NOW BACK TO SURG AGAIN FOR RED, HOT AND SWOLLEN LEFT BREAST (MY NO CANCER BREAST) AFTER CEPHALEXIN-NO LUCK -EXPANDER REMOVED-VERY DEPRESSING FELT LIKE THE CANCER WON THAT ROUND. STARTED LOOKING AHEAD TO CHEMO START DATE AND BAM!!! RIGHT BREAST RED, HOT, AND SWOLLEN BACK ON CEPHALEXIN-THINGS OK- AT LEAST THE INFECTION HAS NOT BURST THRU MY SUTURE SITE. SEE DR. TOMORROW AND I AM ALMOST SURE IT IS BACK TO SURG. TO REMOVE MY LAST EXPANDER. MAYBE THIS WILL HELP YOU FEEL BETTER. HANG IN THERE! MY PRAYERS AND THOUGHT ARE WITH YOU. please-not-pink

REKoz - yes, the black scabbing was very ugly - as was the muscle exposed underneath - the surgeon told me not to look as he dug the necrotic tissue out so I asked them to take a photo. I know that may sound odd, but I'm interested in writing about the experience later and I like recording things. Anyway - today the breast care nurse rang to check up on me & didn't like the sound of what was going on (my breast looks green under the bandage - ew!) so I'm off for a check up. I'll ask about infection/prevention and see what they say...
please-not-pink - expanders sound like trouble to me. I hope I can avoid them. That's the reason I used my own tissue, I was hoping that would be enough and it would mean less interventions. Time will tell I guess. Sorry about your bad luck - I feel that I have got off lightly compared to those undergoing the toxic treatments like chemo - I hope all goes well for you -
Very best of luck to both of you. Kellyx

I too had necrosis with my nipple and had a major infection that took months to deal with. Most of my nipple was salvaged but we had to pack the area and I was on antibiotics forever. I also have drainage problems. I just had an exchange to silicone implant and had a drain put in because last time I did an exchange there was so much fluid. After being in for a week, it was pulled and guess what? Filled up again and the drain reinserted. Unfortunately, some of us have such healing issues. Sometimes it can get to be so depressing. Hugs. Good luck to you.
-
CarolMarie, it's good to have a reality check - I'm expecting my drain to be removed today but I won't take for granted that it's the end of it. Yesterday I saw the breast care nurse who cleaned up the first round of honey "treatment". Although it smelled bad it wasn't infected thankfully and in fact it had done an amazing job of granulating and helping build up the tissue again. So I'm a manuka honey convert - if it has such wonderful healing properties on the outside, it must do magic on the inside too. Worth a try? I've heard just a teaspoon a day can work wonders. x
-
Wow, I wish I had known that when I was dealing with the necrosis. We just did the wet-dry gauze method with packing. Then there was somt type of ointment we used on top once we finally hit the surface after granulation. Once, the granulation hit the top my doctor actually stitched the small area that was remaining. I never had problems and it is fine today. Still dealing with a reinsertion of my drain after I had it for 10 days after silicone exchange. Most times they don't even use a drain for the exchange, but my doctor did it as a precaution. The numbers were so low, but once it was pulled, I filled up with a seroma again. You can't make this stuff up. Sigh PS-Just wanted to add that I know what you feel about not wanting to feel bad because we aren't dealing with an invasive cancer, but you know what it is still depressing to see gaping holes and fluid coming out of you.

I am very anxious today as my right incision has split. I showered yesterday and as I addressed my wound I saw that it was beginning to separate. I have had a black line across my incision since the beginning as I had very thin skin. My PS has hesitated calling this necrosis. Going in to see my PS this afternoon and don't really know what to expect. I hope not to loose the expander as I have read about this happening to other women on this sight. So frustrating as I was just feeling somewhat normal again.I just needed to get this out.
Thanks. Giselle

I don't have any experience with this Giselle, so I can't offer you advice - but I know it must be awful. I'll be thinking of you and wishing you well.
-
Thanks sweatyspice;
Well, I am all cleaned and sutured up. Hopefully I will not have any more issues as the next step is grafting skin from another area.
-
Giselle, glad you are all cleaned up. It makes you feel so much better, doesn't it? I can relate to having a wound separate as well; my back scar (where they took muscle for the reconstruction) has opened up in the middle and it's oozing serous fluid, so in a week or so if it's still oozing I'm going to have to get that redone with sutures etc. And it is frustrating to have a setback just when you start to feel normal again. I might have to have another skin graft (thigh) on my new breast to cover the area of necrosis - so another circle of skin next to the skin graft (back) where my areola used to be - I'm starting to feel like a patchwork quilt. Anyway - good luck to you, I hope it goes well. x

Hi kellyrs, I am sorry to hear about the problems you are having as well. It must be difficult getting comfortable when your back is involved.I guess that some of us are more prone to healing issues. This morning there was blood around my left scar so now I am concerned about what may happen there. What's most frustrating is that I don't know what I should and should not be doing. I am active and have curtailed most of my exercising but now I feel that I should not do anything at all, not even stretch. I have my weekly appointment with the PC tomorrow so we will see. I have followed all directions and just want to get back moving again. I have my fingers crossed for us both.

Hi ladies,
I just found this topic. I had a mastecotomy for my right breast about 2 weeks ago, with immediate reconstruction (250cc filled). I have developed some skin issues for one part of the breast shortly after the surgery. The skin was black for awhile, and it became very dry. The PS said to keep it moist with lotion, etc. Some of it peeled off, and I can see the red skin underneath. Part of it is a bit yellow, kind of like a skinned knee. My PS said it is getting better and it should all resolve, but it certainly looked very worrisome. I am wondering if the ladies who experience necrosis had similar symptoms and how long it took to resolve.
-
Hi Christina100,
(I'm in NYC too right now - working.)
I had the necrosis in both breasts. My PS gave me the silverdine cream and then once it started bleeding and scabbing, I went to wet-to-dry.
Bleeding is a good thing...it means things are wakin' up! My nurse started clapping when she saw the blood and I was scared to death until she said that's what we want.
I had my dmx with immediate latt flap & te March 4th and wasn't able to start chemo until May 29th so it took almost two-three months for my skin to clear up.
Your situation doesn't sound as bad as mine so I don't want to misinform you. What is your PS giving you to put on?
-
Hi girls, I think I am also having a similar issue. I'll explain. I had a prophylactic bilateral mastectomy on July 1st with TE placed at that time. I was only an A cup prior to surgery with larger areola size (so not much skin left to close me up). My PS told me before surgery that I may not get any cc's at surgery, which was understandable, but he did put 100cc in (wondering if that stretched me too much too soon???). So, after my surgery I did notice that the left breast looked "bruised" to me and was reddish, purple in an area near incision. I asked my visiting nurse and she said it was prob just bruising, not worried. Yesterday (the 8th) one week after surgery had an appt with PS and he doesn't like the way the skin is healing in that area. He said the skin may be dying. So, he pressed on it and blood did come to the area so he was happy, but said only time will tell if it will live. He also added that he still can't see the skin under the strips so not sure if that lived. I asked what we would do if it didn't live and he said head back to op room and remove the dead skin and possibly expander if compromised. He didn't give me much more, just said see you in a week. I was given no instructions to help heal the skin, no topical, no oral treatment, no dietary instruction, etc. I have been researching since returning from my appt and I can barely find anything until I found you girls (yay for others helping). So, I don't want to freak out, still hopeful and trying to remain positive, but really wish I knew how I could help it out and save lefty!
Thank you all for sharing and please let me know of any suggestions.
Jen

I had lat flap in March. My right foob has what my PS describes as "staining"........the reddish, purplish, bruisy looking area. He said it should go away after the TE's come out, however, if it gets worse, he may put me on an antibiotic as this is a sign of infection, especially if there is pain. so far, no pain.
Hang in there girls.....the skin is crazy stuff, and it's being stretched, so of course it would seem normal for it to bruise. If you can touch the skin and it blanches then turns pink again, you are more than likely ok.

Pamelajo - I had my surgery Feb.2010 and as you can see from previous posts, I have been dealing with this issue is well. My exchange is next Wednesday and I have been very anxious and worried about it as I still have such delicate, "lightly stained" breasts, particularly the right one. I am feeling better after meeting with my PS. We will be doing the exchange using new incisions at the fold under the breasts. This gives me my best chance for healthy skin. He prepared me for the possibility of not being able to have nipple reconstruction. Sad, but in the scope of things....oh well. I will be looking into the 3D tattoos.
Good luck to you and keep posting. It sure helps!
-
Good luck Giselle. I will be thinking of you. Since I recurred, I can't have exchange surger till November or so, so it's just a waiting game. I notice my staining is worse after a hot shower, so I'm thinking it is just extra blood supply close to the skin surface. I need to make an appt with the PS soon.....but I hate taking off work. LOL
I'm not getting nipple reconstruction. I've decided I like the smooth look and I probably will opt out of tattooing. Not sure about that yet.
-
Pamelajo - The exchange went well and I am happy with the decision to make two new incisions at the fold. I just want every opportunity for my breasts to be touchable again. Nip recon is out and I will deal. But I will be checking out the 3Ds. I do want to have something there. It will make me feel more complete. Every woman a different.
I have noticed that my breasts look worse after a shower. I try not to expose them to too much water and always keep them covered with Vit E.
-
Giselle, I'm so happy things went well. Congrats on being one step closer to nearly normal.
I may tattoo, but no more surgery after the exchange. I just can't handle it.
Did your PS say when the staining would subside?
-
Pamelajo, I understand not wanting to have any other surgery. I haven't had to endure what you have and I am already feeling this way.
My PS and I really didn't address the staining...just focusing on the exchange. I just took a look at my breasts and the left one looks good but the right one looks wounded. It's like a scrape that never heals. Time to use the Biafine emulsion again.Patience!
-
it will heal Giselle. Patience is a virtue, unfortunately, it is a virtue I ran short of a long time ago LOL I'm sure you can relate. I don't mind the staining as it is mostly on the lower portion of my right breast, however......I noticed a patch up on the inner top portion next to my breast bone earlier this week, and that one bugs me cause I can see it when I wear lower cut tops
bummer.
I cannot wait to get these hard things out of my chest and have soft squishy boobs again! Looks like November for me, or possibly December. I have to remind myself that life passes in the blink of an eye, and the wait time on this will too.......but there is that patience thing again.
I have wine to get me through till then LOL

I had my bilateral mastectomy feb 13,2014- had a vast amount of serous drainage after plastic surgeon removed dead skin on my incision lines and an expander was exposed. had surgery april 11,2014 to take out old expanders , put in new expanders and drains . the incisions look great still have sutures in place and my drains continue to drain 40-60 cc per side daily. drain sites without sign of infection. what is causing so much serous drainage it has been a month and my chemo is on hold until the drains can be removed- have to have 2 days of 20 cc or less of drainage. Anyone out there have a clue as to what may be the problem.,
-
May I ask if these occurrences were from skin sparing mastectomies ? I'm not familiar with all the types and may be having a skin sparing mastectomy and wanted to be aware of possible complications. I'm so sorry to hear of everyones pain :-(
