Hi everyone ..had lumpectomy/snb on 10/07 and today got my path repot. The BS said it is good news. Well differentiated infiltrating ductal carcinoma (Nottingham Grade I, MSBR 4/9) size 0.6 cm,margins widely clear.

Comment addest...Pathologic stage I pT1b, NO (i-) MX. She did state that it is ER+/PR- HER- and she said the estrogen was a high percent.

So does anyone know what MSBR 4/9 is and what is the 1b after pT? The report also included that "there is abundant tubule formation(score 1). Which I think is good. Also, there is Mmoderate variation in nuclear size and shape with the presence of prominent chromocenter (score 2). The mitotic rate is low. The carcinoma display luminal micrcalcification. A significant in situ is not identified.

I will be seeing the radiologist onc Friday and then next week see the Med Onc. I will have approx 6 weeks of radiation but don't know much more now.

Any idea on how to break it down? Thanks!!!!!!

Jeannie

I don't know what it means ... but if you call the American Cancer society they can probably tell you.  I called them about my path report and they were very, very helpful.

    Blessings,

Congratulations Jeannie - well differentiated and small has an excellent prognosis!

1b just means that the tumor was no larger than 1 cm -  more specifically, that it was between 0.5 and 1.0.

    ,MSRB means Modified Scarff-Bloom-Richardson which is a grading system as follows;

Tubule Formation (% of carcinoma composed of tubular structures) Score
    >75% 1
    10-75% 2
    less than 10% 3
    Nuclear Pleomorphism Score
    Small, uniform cells 1
Moderate increase in size and variation 2
    Marked variation 3
Mitotic Count (per 10 high power fields) Score
    Up to 7 1
    8 to 14 2
    15 or more 3
The 4/9 is your score based on that grading system. My guess would be that you got a 1 on tubule formation, a 2 on nuclear pleomorphism and a 1 mitotic count for a total score of 4 out of a possible 9 (4/9).

I also had a T1b, well differentiated IDC but 40% of mine was DCIS (also well-differentiated) and my invasive component was called IDC with tubular features.  Was diagnosed just a week after my 49th B-day. I had a lumpectomy with wide, clear margins over 5 and half years ago and I refused the radiation and hormonal therapy, so the surgery is the only treatment I've had for this cancer. I've had crystal clear yealy mammograms ever since. Here's hoping the same for you!!!

I also had some questions about my path report, which I received yesterday, so will get in touch with the American Cancer Society for clarification of it.  This subject had perfect timing for me!
