Here is the calculator provided on the National Cancer Institute (U.S.) website.  The site says the formula comes from scientists at the NCI plus the National Surgical Adjuvant Breast and Bowel Project (NSABP): http://www.cancer.gov/bcrisktool/

Here is a somewhat complicated calculator the author says is based on the Gail Model plus some elements of the National Cancer Institute calculator, plus some other variables:  http://www.halls.md/breast/risk.htm

This calculator is provided by an organization in Australia.  It's a bit different from the others, and it includes more questions about family history:  http://www.nbocc.org.au/risk/yourrisk.html

Even though there isn't much disagreement about the factors that can contribute to increased risk of breast cancer, there is disagreement about the formula that should be used.
