I just found out I might have Paget's breast cancer, still waiting for the biopsy. I feel like it is going to come out positive and I am trying to remain calm, but feel this huge tsunami coming from deep within. It seems most people decided on a mascectomy and then reconstruction. I cannot imagine having my breast removed, I just don't know what to do or how to feel right now, I think I am in shock and still numb...mammogram came out clean but there is obviously something wrong and the doc said she feels it is Paget's but we won't know more until after a biopsy. It is on my left nipple and very painful for about a month now...I am not sure what to do, I feel frozen but know I must keep moving...I am only 49 years old...

Joihappy I am sorry you are going through this. I was going through the exact thing one year ago when I was 44. I had my biopsy on 1/31/08. If it comes back as Paget's go and have a MRI. My mammogram came out clean too but the MRI showed DCIS. Most Paget's are a manifestation of underlining cancer. I don't mean to scare you it is just you want to make sure you know what you are dealing with. I had to go with a mastectomy and still working on the reconstruction.
But on the other hand maybe it is something completely different. Keep us posted on how you are doing and email me if you have questions.

Dear Hannahbannana thank you for you support, I feel like I was lead to this forum as I am pretty sure I am going to need the support. I have read a lot on Paget's breast cancer and realize there is more to it than meets the eye. Almost 95% of all Paget's breast cancers have an underlying cancer, so the fear is already there and I am trying to figure out what I have. I have contacted the Huntsman Cancer Institute here in Utah where I live and am waiting to hear back from them. I want my biopsy, MRI and all testing and treatment to be done by someone who knows what they are doing. Unfortunately and fortunately I work in the medical field as a medical office administrator for neurosurgeons so I think I know too much and yet am glad for the connections. Again thank you for your support and for you quick response. How are you dealing with the mascectomy without me prying too much? JoiHappy

This forum is a wonderful place! I found it as soon as I was diagnosed and it has been a great please for information and support. I don't come by as often as I used to but stop by every now and again.
Don't worry about prying too much with questions I remember how others sharing what they went through was a big help for me. I am coming up on one year in March for my mastectomy and I have been very blessed. I did not need to have any treatment which made things a lot easier for healing and reconstruction. It was rough at first but I knew right away I was going to have reconstruction so I focused on that. I still have more surgery so the reconstruction is taking longer that the time between diagnose and surgery Ask any question you may have.

How are you doing now? I am noticing that, although statistics state that this is more common in older women, it's become more common in younger women as well. Are you on any medication? How big was your wound? Sorry for prying. I am just feeling very helpless at this point and any encouragement will help. Thank you for sharing.

Hello abick, no problem asking questions or prying I know how it feels. I am so sorry you have to be going through this. I was 44 last year when I was diagnosed and am clear one year later. I do take Tamoxifen for another 4 years because I still have my natural breast on my left side. I had a full mastectomy with removal of the nipple because of the Padget's. I also had DCIS underlining which manifested itself as Padget's which is what save me because my mammograms came back clear. If it wasn't for the Padget's I would never had known. So my scar runs horizontal but is stretching from the reconstruction. I am in middle of the process. I hope to have surgery on the left breast to give it a lift to match the implant on my cancer side.
How did you find out you had Padget's? What are they suggesting you do? Let me know how you are doing and ask any and all questions you have.

Hi everyone...I am new to the site, and very nervous that I am in the wrong place (not very computer savy about these things), so please forgive me if I do this incorrectly....I need some advice about Paget's or Paget-type symptoms...I don't know if I should be concerned or not. A while ago I developed that itchy scaly symptom that makes you insane - scratching makes it worse - the creams don't help....it went on for months and I wasn't concerned until I started to have the discharge from the small areas and occasionally the nipple. I went to the doctor, he prescribed a cortisone cream and diagnostic mammo. The cream didn't work and the mammo was inconclusive. After 2 more rounds of stronger creams, oral steroids, an ultrasound, MRI and finally a punch biopsy we still had no answers. The final straw was that my breast was shrinking. No one that I saw could explain to me anything that was happening and I think I became a source of aggravation for my docs. I just wanted to know why, and they couldn't explain it - this seems to make them very angry! The last visit I had with my doc he told me he didn't know what or why and maybe I could go see a plastic surgeon to let them fix it. I haven't been back to the doctor since. Eventually the itchy scaly symptoms subsided - the size is still smaller though, and I thought maybe it was some crazy something...well a week ago I noticed the itchy tenderness coming back, and the skin is starting to scale around the nipple....I know where this is going, I just don't know what to do. Am I crazy?

I am sorry you are having to go through all of this. I know it is not easy but keep pushing because there seems to be something going on. Can you go to another doctor or a dermatologist? That is who found mine. Paget's symptoms can be where it comes and goes which mine did, not trying to scare you. But please try and push some more. I hope this works out well for you. And no you are not crazy

I was very lucky in that I did not need any treatment after my mastectomy. They were able to get everything through surgery.

I have a question to ask. Hope you can give me some advise. I have itchy nipples of both my left and right breasts for couples years. My left breast was diagnosised of DCIS last year through mamo shown calcification. I had a single mastetomy on my left breast. I have itchy nipple for quite a while. After I scratched, the itch would stop, normally it will be more itchy during winter. This winter, I put some lotion on my leftover right breast, no itch. Right now it's May, I think it's late Spring and warm, so I stopped put on lotion, I noticed the itch is back with a little scale sometimes. It's not itching all the time or all day long. I don't notice any redness and rash on it.
Last year I have mamo and MRI shown my right one is ok, this year in Jan my mamo show my leftover right one is clear. I am going to have another MRI in late June.
Do you think I should ask my doctor for a nipple biosy? After DCIS last year in my left breast, I want to be more careful with the other one.
I am going to make an appointment with determologist after reading your ladies' posts.

I requested a mri at my one year check up last March and happy to say the remaining left breast is clear. Coming from the only way I knew I had cancer was because of Padget's of the nipple. This was found from my dermatologist. With that said I would discuss everything you are feeling with your doctor. Track when it is itchy and when not or if it is flaky. Paget's can come and go but do go thinking the worst but as you know it never hurts to be careful

I have had a horrible rash on my nipple for about 9months now. When I went to my doctor all they could tell me is they didn't have any idea what it was so I didn't think much of it. Now it is 8 months later and my nipple has become much worse. My nipple and the surrounding area has red raised bumps on it, itching, burning, cracking, oozing yellow fluid and more. (Looks like bad ezcema). I have used steriod cream that only took it away while using 3 times a day. When I stop the rash comes back even worse then before. I got online and starting reading, found Paget's cancer. Now I am scared to death and have a really bad feeling that I have cancer. I had my annual mammogram several months ago that came up clear. My mom had invasive ductal cancer about 2 years ago so I have gone in last two years to have the mammograms and thought that was all I needed to do to make sure I didn't have cancer. I wen't to my obgyn yesterday to have her look at my breast, she said possible Paget's and now I have an appointment next Wednesday to see a breast specialist. I guess they are going to take a biopsy. Does any one know if that will take off most of my nipple? I am very scared to have this done! I was also told that most people don't get this at my age. I am 36 years old. If any one has any advise or suggestions on what I should do please I would love to know.

Dilch welcome to the site and sorry you have the need to be here. I had some of the same problems as you did and had a biopsy. They take a very small amount that you won't really know it is gone. It is a bit painfull since the nipples are very sensitive so even though they numb it up a bit after it wears off it will hurt a bit but some over the counter pain meds should be fine. All in all it is very simple and I am sure you will do fine. I agree that you are very young, I was 44 when I had mine last year but no matter what age one should have thing checked out.
My thoughts are with you and let me know how things turn out.

I have your read replies, I hope that you are still reading this site, I cannot tell how this has helped.
I have recently been diagnosed with Pagets disease, I had an operation to remove a lump in my duct because my breasts was leaking last year October 2008, (my right breast), the nipple would not heal, I went back several times and was told that it takes time to heal, my nipple was extremely painful and yellow pus and blood was coming it from it, I was given different courses of anti biotic and told to use nipple cream.
It would heal for a short time and then start crusting and bleeding again. Through being sent an E-mail on Paget Disease, I requested to see my consultant again, he then decided to conduct an ultra scan just to pacify me, and they found a lump, it was extremely small but it register some cell changes, whilst the surgeon was performing a lumpectomy the nipple started oozing and he decided to conduct a biopsy of the nipple and that is how my Paget's was diagnosed. I have to have a mastectomy with reconstruction LD with implant in November 2009, what is the healing time for an operation like this. How long from being diagnosed to operation?

Hey there JC123 sorry you have to be on this board with your situation. Your ordeal sounds like a long one that should have been shorter. Sometimes it is frustrating when the doctors don't want to listen and they should have been concerned at the oozing. I started with my regular doctor who had me get my mamo and an ultrasound which both came out clear. The thing that bothered me was the doctor who did the ultrasound said not to worry about the nipple unless it oozed. My md sent me to my dermatologist when I said it hurt too much and she did the biopsy.
My time from diagnoses and surgery was 4 weeks things just seem to line up real fast. I am the type of person who needs to know what is going on and have a plan of attack. So once I found out what was going on I concentrated getting everything lined up and researched everything. For me the not knowing was the hard part.
Are you having the mastectomy and reconstruction at the same time? I did only the mastectomy along with 7 nodes be removed at first. I was out of work for 2 1/2 weeks from work but went back part time during the last week of it. Of course I was sore for some time. I then went back 3 months later for an expander to be placed. That hurt more than the mastectomy because they put it in behind the muscle and my doc filled it up quite a bit at first. My doc does things in stages and likes to make everything heal before moving on. I had my good side lifted with a small implant and had my right side fitted with 3 implants. (3rd time is the charm but that was because of the size I wanted) Having it done all at one surgery probably take a good couple of weeks of recovery and then take it easy.
Hang in there you sound a lot like I did 18 months ago. Scared and unsure of what is happening and what will be next. You have gone through a lot to get where you are now. I spent a lot of time on this board and everyone was really great. Keep me update and write anytime you want I have this topic flagged to email me when someone post on it. I have you in my thoughts and ask all the question you need I don't mind.

I am having reconstruction and reduction at the same time as I am on Warfarin because I have an artifical heart valve they are afraid that because I have major surgery before and they do not want to keep on opening up.
The waiting is killing me. I don't know what questions to ask, I cannot even remember the plastic surgeon name.

I have just had my operation cancelled. I have a heart condition and they forgot to consult with my cardiologist. I feel so stressed, my mother lives with me and this is not helping her either. My daughter is snapping at me and my husband attitude stinks, he not very considerate. They are making changes in my job and could not wait to tell me after my operation, I don't know how much more I can take.

JC123 I am so sorry you have to be going through this after all you have been through. I had one of my operations canceled the morning of. I was getting ready to leave the house at 6 when they called to say the nurse was in the ER and I would have to reschedule. It is very hard to hear because your mind and emotional state depends on it. Are they going to be able to reschedule once they talk to your cardiologist? It always seems that stress always piles up when you already have enough. I remember when my hubby was in the hospital twice within 9 months as much as I knew it wasn't his fault but it was hard on me and I would get a short fuse. Not to excuse family members attitudes but I hope things mellow out for you. Is there any way for you and your hubby to check into a motel room for one night and have a little alone time? Sometimes having a change of scenery can do wonders. I hope things get better soon keep in touch

I saw "pagets" and popped in, although so many of the posts are old and I may have even entered into the thread at one point. I was diagnosed with pagets now three years ago after visiting FOUR doctors! Thank God I stayed on it, even though a biopsy said no cancer. The needle was a smidge off, I guess, and got a clean area. I wish I was your stage but the delay with all the doctors left me at Stage II. I do not know for a fact that the delay put me at a later stage, but I do know I walked around with that itchy awful nipple for 12 months. I was told to change my laundry detergent! The nodes were clean! I had chemo and radiation.
Anyway, I have stayed on the boards with "Illinois Women Facing Cancer" and it is a great place for me.
My questions is have you ever discovered why you and I presented as Pagets? Once they finally found my cancer, they said Pagets was of no further concern. It actually saved my life, in fact, in that nothing showed on the mammography and even barely on the ultrasound. Yet the tumor showed up as 4 centimeters on the MRI. That is like big!
Why do some of us get Pagets and others do not. It is rare, but what actually is it. Pagets itself is not cancer. You can have Pagets without cancer (that is rare); why did you and I have Pagets in the first place when most woman just have the cancerous lump.
Just wondering if you have figured any of it out. I just hope I can find this place again. The group has so many threads.

Paget's of the nipple IS cancer. The difference is :
Yes it's rare, it can be found by itself but more commonly is only one breast and with an underlying secondary cancer. When it is the only cancer it has an incredibly good success rate for treatment. When it is with a secondary (transformation is the correct term.) the secondary is the focus of the treatment and drives the decision tree. There are some theories as to why but nothing definitive yet.

Yes Paget's is cancer and I had DCIS in addition. My doctor felt that the DCIS manifested it self out as Paget's Yes it is rare which is why it seems to be harder to detect and doctors seem to think it is something else. I had a great GP who sent me to my dermatologist when my mamo came back clear. She knew something was wrong and after treating for a staff infection a biopsy was done. It hurt so much I was past the point of not finding an answer. I am grateful for my Paget's (I know sounds wrong) because if it wasn't for that I would never had found out I had DCIS because all my test came out clear. It was only found when they ordered a MRI after finding Paget's. My oncologist said if it went undetected it would have become invasive within 6 months.

Hi guys, I am having symptoms of Paget's, you can read my previous posts for a full explanation. The only symptom I have is the flaking/peeling/crustiness of one nipple. The symptoms come and go. I had a punch biopsy performed however there was not much flakiness the day of the biopsy. The results came back as a benign fibroepitheleal polyp...
I am questioning this result because it is a skin tag and I do not have any sking tags or any sort of growth on my nipple.. my follow up with the breast surgeon is on monday....
what do you ya'll think? what would ya'll do if you were me?
I plan on waiting to see what the breast surgeon says to see if it makes sense...or explains what is happening to my nipple...
What type of biopsy is best? What kidn did you have and was it accurate?
Whatever is going on, I would like to catch it early and make a 100% certain it is nothing serious.

Talk with the BS. It is pretty rare, so if the pathologist looking at it has not seen a lot of it, they can miss it. See if the BS feels your slides should be double checked. IMHO

I have been complaining of right nipple crust for 3 years till now, I am only 34 and have no family history of BC. I am familiar to fibroadenomas in both breasts.
The worry is eating inside me and I am unable to enjoy my life, which is causing lots of issues and fights with my husband. I am unable to take a decision. I did two FNAB 10 month after the issue started. It all came clean. I have an underlying movable math right underneath my areolar. It is like a disc or something. I have crust in my nipple. No other complains such as lymph nodes, or skin changes in the breast or shape. My nipple is a bit pinkish in color and looks puffy. No bleeding or oozing.
I visit my surgeon every month not because of complaint but because of worry. I was told by 7 doctors that this is fibrosis or papilloma which mimics paget's. I was told if it was paget's or BC you could have no lived this long without developing further symptoms. I was advised not to remove it since treatment means that they will do a nipple excision and areolar which will cause me a psychological pain and discomfort.
I lost lots of my years in panic and worry, I am unable to live a normal life and afraid to take a further step. The doctor says its my call if I want to remove it, but his recommendation not too.
What if he was wrong, or had miss-diagnosed me! Do I go for excision of the nipple and areolar complex!
I ma really afraid, and would like others who experienced paget’s to help me with the symptoms and share their experiences. What else could be similar to paget’s and is a benign disease, anyone knows. -
By the way, I just want to know what changes happen to the breast and nipple when Paget's disease. Is it true that the nipple changes with ulcers and openings that could bleed. What if the nipple is pulled outwards with no other symptoms. Indeed paget's differ from one to another, but at least there are common symptoms that are shared. Is tue that someone can live with paget's for more than 5 yrs with no symptoms?

My sister had Pagets. She had a kind of raw, irritated area on her nipple that would drain a little causing her bra to stick. It was sore to touch and her nipple became flat. Her mammogram showed thickening of the nipple and calcifications beneath. Have you had any biopsy of the nipple yet? The biopsy for Pagets' is usually a simple punch biopsy that can be done in the office. I would think that it would have worsened over the past five years if it were malignant. Hope you have a definitive answer soon.

I haven't done a punch biopsy what they did is a scrap biosy (they scarp the head of the nipple with a scratcher) and they digged deep into the middle of the nipple to take a sample tissue from a suspected lump underneath. another biopsy was done and it was from the lump underneath the nipple. It all came clean.
I once felt that there is a yellowish (goldish) semi-sticky thing that came out from the erolar, and when visted the doctor he told me it is probably the montgomry glands due to the infection. It was dry and had no blood or anything underneath it when the Dr. poped it and it was clean. That's why he says it is mongomry gland in the erolar.
I was wondering, how could the nipple suddenly become flattened? Does any process happens gradually before it completely becomes flattened? Does it necessarly bleed.
What about the crust, mine build crust occassionally, but when I apply vaseline it does not. If I do not use vesilne it becomes dry and builds crust that falls on its own. I read about nipple adenoma, and other types of adenomas and florid papiloma where it mimicks paget's disease, any one has a clue.
