Estriol Vaginal Cream

Hi all,  Does anyone know if estriol vaginal cream can be purchased over the counter or does a gynocologist have to give the prescription?  I recently asked my gynocologist for the estriol  but they recommended estrace (0.1mg).  The nurse said they are now using estrace at MD Anderson.  Do I have to go to a special doctor and compound pharmacy to get the estriol ?

    Tweeti, we have had a big debate here about hormone replacement therapy. I am considering it myself, but would not do it without a doctor's help, and I will only do bioidentical hormone replacement, as the synthetic HRT does not react the same way, according to studies, and has been linked to promoting cancer. Two books that really explain it are:

    Natural Hormone Balance by Dr. Uzzi Reiss 

    "The 30-day Natural Hormone Plan" by Dr. Erika Schwartz (she has a couple of other books too)

    To find a doctor who really understands how to prescibe and monitor BHRT, ask for recommendations from your compounding pharmacy. This is how I found my doctor and I am relieved to have someone who understands all the ins and outs. I strongly believe this is the wave of the future for breast cancer prevention, and for healthy aging in general.

    I never considered it before but suddenly I can't tolerate the dryness and insomina.  I have tried over the counter products like Replens etc, but they are not giving me much relief anymore. Now I've developed atrophic vagintitis.  I've was on Arimidex for over 5 years and got off for a while but recently started again, maybe its just time to stop.

    Tweeti,  my gynecologist prescribed Estrace and Vagifem for me (to help reverse vaginal atrophy).  I'm glad to hear you say they are prescribing it at MD Anderson...but for what/who exactly...do you know?  I'm still nervous about using it (estrogen + cancer).  In fact I just quit because I didn't feel it was repairing me to the point that I could have pain-free intercourse...maybe my dose was too low.  Blood tests showed while at that dose I am still fully menopausal.  (I was thrown into menopause with chemo...and also feel like I've aged 30 years overnight by taking Aromasin for the last 2 years.)

    Does anybody else have more to say about HRT reversing vaginal atrophy?

    I, too, am a bit worried about experimenting with hormones on my own, but from what I've read, estriol is likely to be less problematic for ER+ BCers than is estradiol, the type of estrogen used in Estrace and Vagifem. In fact, one study showed that Vagifem did elevate serum estradiol levels -- not a good thing (http://annonc.oxfordjournals.org/cgi/content/full/17/4/584). However, estriol, being the weakest estrogen, is also the least likely to be effective in reducing symptoms. 

    That said, I have run across a few sources of estriol products that don't require a prescription. PM me if you're interested.

    Lifes - We were talking about atrophic vagintitis and Estrace when the nurse told me about MD Anderson using it, so I  assume for the same reasons.  I asked for the least possible dosage, and they recommended the .01 mg.  Vagifem has higher estrogen, and after reading the study posted above that really concerns me.  I wonder why the oncologists are sayings its ok.

    Good Lord I wish all the doctors would get on the same page!  This is what happened to me.  I was given a script for Estrace by my onc and was told it was safe to use to help my poor vaggygy out.  I was back on Tamoxifen at the time and was also leary of using it.  Then I was put back on an aromatase but no one told me not to use it until I asked about a recent study.  The study basically said you can use Estrace or Estring while on Tamoxifen because it attaches to the estrogen but not with aromatase inhibitors because it conflicts with the way they work.  When I asked I was told to stop using it.

    ibee - I know what you mean.  I wish they were all in agreement.   When I went to pick up the Estrace the pharmacist didn't want to fill before calling the doctor.  He said it was in conflict with the Arimidex. Now I'm not sure I should use it.

    I used Vvgifem for 5 years and noticed the warnings getting stronger and stronger about a possible connection with breast cancer. My old gyn kept reassuring me it was not a problem. Last May I was diagnosed with breast cancer and there is no history of bc in my family. I no longer use Vagifem but had a big problem with vaginal aatrophy & urinary infections.  New gyn put me on Estring saying it was the lowest possible dose of estrogen available. I reluctantly use it and it has greatly improved my symptoms.  Some gyn recommend Estrace cream but not the full dose. They have gals use just a small dab on their finger to apply the cream.  sometimes I feel damned if I do and damned if I don't!

    I have ordered myself Ovestin cream. The lubricant my doctor recommended did not do anything and I am worried that if I leave it any longer it will be too late for recovery.  I decided to trust Dr Lee on this one.  He claims that estriol is not involved in breast cancer.  I take Tamoxifen and hope it does its job while Ovestin is doing its.  Wish me luck.

    Gals,  my gyno and oncologist think it is ok to use small dose Estrace and Vagifem because they say it does not enter the blood stream.  My doc checked my hormone levels while at the dose I was on and I'm still menopausal.  Don't know if there was an elevation of hormones...but still menopausal tho.  I know the Aromasin provides no protection against therapeutic estrogen...I don't think the docs are sure of how vaginal use of Estrace and Vagifem can result in a recurrance.  But my gyno's sister had BC...and her sister uses it.  My gyno's other patients use it...the dose she recommended to me was start up - daily use for 2 weeks...then 2 x a week for 4 weeks...then 2 x a month continually.  Yes...the Estrace is just a pea-sized dab in the outer entry area...the Vagifem is inserted vaginally.  Doing all that for 6 months...I was still menopausal.  It helped some...but not enough.  I wonder if I needed a higher dose.  Has anyone heard of Vitamin A & D ointment for the vaginal atrophy?  What's the deal there???

    Life, I was using vitamin E suppositories several times a week, alternating with Replens, and that worked very well  for me the entire 5 years I was on Arimidex.  Just recently I needed more.

    My gyn said that Replens & vit E only acted as a lubricant. They do not solve vaginal atrophy.  Estrogen products plump up and improve the vaginal tissue.

    Hi everyone. I am an FNP with menopausal concerns. I do not have breast cancer, but I have been researching the whole menopausal, bioidentical hormone situation for quite a while. The conclusions I have come to are that estriol is safest. It is also weakest. The vaginal cream is available through the Internet without a prescription. Also, there are sources for small doses of testosterone, which they say helps with libido issues. I am at the point where I'm going to try the estriol and testosterone vaginal creams. I'm hoping they work. I don't think they increase or exacerbate breast cancer rates. I hope not. I can update you after I've tried them. Good luck to everyone. It's frustrating, to say the least.

    I go to a doctor that is a DO instead of an MD and she prescribes estriol vaginal cream that is compounded from a compounding pharmacy.  She has informed me that it is safer than Estrace which most gynecologists prescribe.  Most insurances will not pay for estriol because it is considered more like a supplement even though it requires a doctor's prescription.   I have been using it for several years with no problems.

    I've tried estrace, which was prescribed by my GYN and covered by my insurance. I've found it does work, even though it can be a bit messy, so I just use a little mostly around the vulva and clitoral areas.  

    After that,  I found some estriol suppositories which I ordered online, which contain estriol, cocoa butter, beeswax, and vit E.   They come in 1 or 2 mg doses.       Just recently, I started going to a holistic type Internal Medicine doctor and she prescribed an estriol/testosterone cream, which I had to get filled at a compounding pharmacy, so of course it was not covered by insurance.  She also tested my hormone levels before and after a few months of using this product and estrogen levels never rose above postmenopausal levels. 

    I have to say, that out of the 3, my favorite product has been the estriol suppositories which I found online.  Because of the Vit E in them, they leave a coating on the area, so the product stays more in place to do its job, instead of leaking out and making a mess like the creams.  And from what I've read, estriol is supposed to be the most effective estrogen product for the genital area.

    Anybody here with recent experience talking with onc/gyn re: estriol cream use and safety for BC patients? Am trying Replens now, and it doesn't seem to be helping enough. Any insight appreciated. Thanks.

    I was just about to try this estriol cream as I was desperate, and then learned of a procedure called ThermiVa. I had the 1st of 3 treatments 2 weeks ago and it has been remarkable. Something you should consider as it's not a medication and certainly NO more estrogen to add to your body. You can simply google it...ask any questions and I would be glad to help :-) Take care!

    I had vaginal atrophy as a side effect of anastrozole. I started using Premarin vaginal cream, but my drug coverage paid nothing towards it. The tube cost me close to $300. My Pharmacist told me about Estriol vaginal cream. The cost for it is $30. per tube. As you can see, a considerable difference in price! Now, the all important question. Which one works better? My Oncologist requested that I use .5 grams three times a week. I used Premarin for two weeks and was instantly impressed. I then used the Estriol for two weeks. I honestly felt no significant difference between the two. I should add that I also use Astroglide Diamond Silicone during intercourse. Together, sex is comfortable, and once again enjoyable. Hope this helps anyone with atrophy

    I have had two, of three, ThermiVa treatments. I noticed mild to moderate positive results so far, but am hopeful for more—my understanding is that it can take awhile for full effect. I still have thinning, red skin in the inner labia area, so my doctor also put me on estriol. I've only been taking the estriol for two days, so not sure yet on how effective it will be, but happy to give it a try since it's relatively safe and inexpensive. I'll have my third ThermiVa treatment in about 3 weeks, so I'll keep you updated as things progress. Fingers-crossed it works!

    MDA is pretty conservative regarding products such a this...so if they approve it, I would really consider it rather safe.

    I started a small compounded vaginal dose of E2/E3 awhile back( Bi-Est)...did the job as they say...also helped with the most severe of the flashes....I am 6 yrs out of menopause, and the ole ovaries may not be useful at all anymore. I was told by a GYN that even in the early menopausal years, the ovaries still produce small amounts of hormones...by age 55, they really head downhill.

    Last hormonal panel showed a whole whopping E2 level of a 3..... Now my testerosterone could light up Houston...it is in the 60's....and that is while on a good size dose of Metformin, which is known to lower T levels. It has been suggested that those T levels are due to my weight lifting...by my BS. He might know, since he is into weightlifting

    i really wish ovestin were available in the u.s. apparently Wyeth, makers of Premarin and Prempro, exerted pressure to ban prescription estriol here. Oddly, it's being sold over the counter. I haven't found any estriol product available here with an applicator or in the form of an insertable tablet or suppository. I'm wondering if it would work too solidify the cream by freezing measured dollops to make it easier to use vaginally. I wouldn't use an estradiol product with my breast cancer history.
