Moody, here are two graphs from a paper that discusses the prognostic value of Oncotype scores. The graphs are "Figure 2" from this article: J.A. Sparano & S. Paik: "Development of the 21-gene assay and its application in clinical practice and clinical trials." Journal of Clinical Oncology Vo. 26 No. 5, Feb. 10, 2008; pp. 721-7228. The article and the figure are copyright-protected, with access available only by subscription.

Part A is a plot of the % of patients who had not developed metastatic BC within 10 years after diagnosis (or surgery?), with Oncotype scores grouped into the 3 categories of "low risk", "intermediate risk", and "high risk".
Part B shows the 10-year rate of distant recurrence, with Oncotype scores plotted as a continuous variable instead of in groups. The little black lines on the horizontal axis represent each individual woman's Oncotype score. It's obvious that there were many more scores in the "low risk" category than in the "high risk" category, in the study that validated the scoring system.
To find out (roughly) what recurrence risk corresponds to a particular Oncotype score, find the score on the horizontal axis and draw a line from that place up to the blue line. Then, draw a line from where you are on the blue line, over to the corresponding place on the vertical axis. This will provide a rough figure--I suspect in the lab at Genomic Health, they plug the numbers into a mathematical formula, which would be more accurate.

[Edited to add: All the scores and recurrence risks reported with Oncotype testing assume the woman will be getting 5 years of tamoxifen. The graphs above are also for women who've taken or are taking tamoxifen.]

Just wanted to let you know I found my copy of my oncotype test and the score of 33 actually equaled a 22% chance of recurrance. My onc has said 'about 25%,' so that number got stuck in my brain.:-)

This is my first post to the board. I am so grateful I came across these discussions. They are so informative and supportive...just what I needed. My oncotpye score was 29 - 19% recurrence. I start chemo (TC x 6 cycles) on Jan 2.

All these women, sharing our experiences and support will help you more than anything, especially comforting to know that every emotion or thought you have is NORMAL!!!!!!!

what if I don't have an oncotype score? how do i know my risk%? i'm meeting my oncologist tomorrow....will they have risk #'s just based on my tumor size, grade, etc.?

Hi lisalisa Your onco should give you some "statistic" for your chances of recurrence whether he uses Oncotype DX or Adjuvant Online or some other resource so that you will have enough info to decide what tx you wish to do.
Yes, it analyzes among other things: Size, grade, hormone, HER2/Neu, histolic factors (comedo,etc) etc.

By the looks of your cancer staging, tumor size and node involvement, I would venture to say you will be looking at chemotherapy. Now I am NO doctor, just a "layperson" guess. And I would also guess you are looking at several months worth (prob 2 diff cycles) and radiation plus possible hormone therapy.

i've already started chemo. i will have 6 rounds of TC. i also know that i'll have radiation.
so, i don't need an oncotype score to figure that out.....just wondering how i get the % numbers.

thanks so much moody!!! i appreciate it. i'm seeing my oncologist tomorrow so i will ask.
i will admit that i purposely didn't ask before as i wasn't sure that i wanted to know. but, now that i've been thru 2 surgeries, a port, started chemo....the stress of all that is sort of over. i can move on to longer term LOL!

Below I cut and paste from the Oncotype website how the Oncotype reaccurence score is calculated. You can see the weight given to HER2 and ER/PR. the other big factor is the proliferation grouping...
21-Gene Panel Used to Calculate Recurrence Score Result
The Oncotype DX assay gene panel was selected from the published literature, a genomics database and experiments based on DNA arrays performed on fresh-frozen tissue. The Recurrence Score result algorithm was derived from 3 studies of breast cancer patients and then was clinically validated in multiple independent studies.
- The Recurrence Score result is calculated from the expression of 16 cancer-related genes and 5 reference genes used to normalize the expression of those cancer genes
- In clinical studies, the 16 cancer genes demonstrated a consistent statistical link to distant breast cancer recurrence, as well as robust predictive power regarding chemotherapy benefit

mattscot, pretty impressive! Thanks for posting this. Man with all the info on this thread, we will become the "experts" in recurrence scores. Shoot, I bet we have more info here than our docs do! LOL okay maybe not, BUT going to see an oncologist, armed with sound knowledge is certainly helpful.

Moodyk13 - I just got my oncotype score...23. The nurse told me that the recurrence % was 15% and that is with tamoxifen. This was over the phone, and all she would tell me as I meet with the Oncologist 01/07.
I don't think my info is with my signature, so here it is:
44 yrs old, dx 11/7/08 IDC er/pr+ ,her2-, grade 2, brac1/2 neg., lumectomy and snb 12/01/08 - 0/3 nodes clear, clear margin of IDC with 1mm remaining of DCIS (rexcision pending) chemo type?

My dx is also on my sig. line.
I was an onco score of 20. I thought I was 13% without tamoxifen....with tamoxifen it was 8% recurrence. I didn't do chemo but am on tamoxifen and don't recall all the numbers now (I am almost 2 years out). I'm too chicken to look back at the reports. I'd like to live like an ostrich for a bit longer.
My DH thinks it was only like 3% recurrence but I recall that doing chemo would only give me a 2-4% benefit, so I opted out.
It's a crap shoot.

Thanks wallycat I got you added to the roll call. For me tamox was only gonna benefit 4% so I opted out of it. I did 4 DD A/C tx and am half way through Herceptin then I will be 92% !!!

My Oncotype score was 28! I thought that was WAY too high! So I opted for 4 tx of Taxotere & Cytoxan, which I completed Oct 9, 2007, and I am now a year into Tamoxifen.
If you add me to the list, is this for statistics?? Just wondering...

Here is my info....46 years old, oncotype dx 15, reoccurance was 9%. 1.2 cm, stage 1, grade 2, ER/+/PR+, Her2-. 0/7 nodes. Have had my 4th of 8 rounds of CMF. Will be followed by rad and tamoxofin.

linda I have thought about adding at least things like ER/PR & Her2/neu status as well as tumor grade. I may go back and add it. Your score does "bite a big one"!!!!
scarp hello, thanks for sharing your info with us. I got you added to the roll call. You are really being put through the ringer for a score of 15! I fact w/ only 9% recurrence I am shocked they have you doing all that. My score was 36 and 25% recur + Her2/neu ++ and my treatment wasnt as detailed as yours. Bless your heart!!!

My score was a 20 - on the fence - so decided to join the TailorX clinical study and the computer decided my treatment - mast, no chemo, just tamoxifen. It's been 2.5 yrs and I'm still cancer free. I saw my onc every three months but have now graduated to 6 months. I will be on this study for 20 years.

Hi - my score was a 22 with a 14% chance of recurrence. At first my oncologist said I'd be fine with just Arimidex (I'm post-menopausal) but then he realized my Fish report never arrived. After it was faxed to his office he immediately changed his recommendation because it came back Her2++++. However, he thought it overkill to put me on the traditional chemo with Herceptin since it can cause heart damage (the A/C chemo) so he put me on 4 months of Navelbine (which is normally used for liver mets) and I continue for the remainder of the year with Herceptin. He said this will reduce my recurrence risk to 7%. I'm hoping for 0% recurrence since I also opted for a mastectomy even though my tumor was only .9cm large. Only time will tell................

Just wondering...I read that Genomic was going to start giving additional information regarding HER2 status in the fall of 08. Have any of y'all received that? My test was almost a year prior to this announcement in Sept 08 (scored 19).
