DCIS & IDC-What Were Your Grades?
Im Curious..
So I am looking for those who had DCIS & IDC.
I want to know what your grade was of Each.
My DCIS was grade 3. My IDC was grade 1.

My IDC says grade 3, but my DCIS just says "high grade". I see the onco today for the first time and hope to have him translate for me!

I have both the DCIS, Stage IIa, grade 2 and IDC same - also have the rare tubular formation and cribiform - T 3

I have both DCIS and IDC and I never thought to find out the grade for the DCIS. I have an appointment tomorrow and will ask my onc.

I forgot to mention - I am on second round of chemo after left mastectomy - the mastectomy showed another area of DCIS, which was not graded as we already had Dx. My doc told me they call it a multi-centric cancer, plus two individual cancers - DCIS and IDC ,two tumors to far apart to be considered same - 1.8, 1.3. I thought IDC stemmed from DCIS - but apparently they consider separate. The Onc combined the sizes of tumors: 3.1 and that is why I am doing chemo - chance of reoccurrance is 25 -30% without chemo. No nodes, which seems very positive. Fear of reoccurance or met is weighing on me today.

I don't think my dcis was graded either....at least I can't find anything about it. But, my IDC was stage 1, grade 3.

Pam, my 1.8 cm tumor was almost all IDC, grade 2. There was a bit of DCIS associated with the IDC, and it was "high grade."

hi there...
i had DCIS Grade 1..and i had 2 tiny areas of IDC 1mm and 4mm..both of those were Grade 1.

and pagets
all were her2+, the idc's were estrogen positive

I had a .8 cm IDC tumor, intermediate grade. My DCIS was more extensive, also intermediate grade. I also had ADH on the resection, multiple foci "suggestive of extension of ductal carcinoma in site into lobules". I haven't figured out what the extension into lobules means.

Multi-focal, multi-centric here.
Primary was IDC- 1.7cm, ( 20% DCIS) and both it and the IDC were grade 2.
Secondary was ILC-1.0 cm, not sure the total LCIS, but both were grade 2.
Vision4utoo? I'm surprised that they combined your tumor load for you tx and staging. It's not normal, and they usually do just the primary for tx purposes. Even the oncotype does not use both tumor information when doing their RS, which made me a bit uneasy and not as confident in their outcome. Where were you seen, do you mind me asking?

My tumor was 2.1 cm with 20% DCIS. My DCIS was low grade and my IDLC (both ductal and lobular) was grade 1. However, I had one positive node, go figure.

My DCIS was grade 3 and my IDC was also grade 3.
In my path report they said the tumor was 1.7 and extensive DCIS. I alway's assumed that the IDC was 1.7 and never thought to ask how far the DCIS went. Wonder if it matters?.............

My tumor that was about 1 cm with 60% of it being both histologic grade and nuclear grade 1 IDC NOS with tubular features. The other 40% was grade 1 DCIS cribriform type. ER strongly positive and PR positive. Her-2 negative (1+). Ki-67 5%. No microcalcifications. No necrosis. No venous or lymphatic invasion. Only one node removed with the SNB and it was negative. The tissue removed in the lumpectomy measured 7.5 cm x 5 cm x 3.0 cm.

Hi dejaboo - I had 5mm IDC (grade 1) and intermediate DCIS with a total of 18mm being removed at lumpectomy (this allowed for wide margins). Jaydee

My understanding is that DCIS Ductal carcimoma in-situ can develop into IDC invasive ductal carcinoma. so if you have IDC it happened because the DCIS started growing outside the duct. I believe most path reports for IDC will show some DCIS component. Although you can have multiple areas of either DCIS or IDC that aren't related to each other, for example one located at 1 o'clock another at 7 o'clock.
my in-situ carcinoma was grade 2, with invasive DC going to the margins.
When I had the resection there was a focal residual DCIS of 2.5mm of "low nuclear grade, with no invasive DC found. Kind of shocked me that no invasive was found since it went to the margins in the first lumpectomy.
Jaydee, I have read that a wide margin is great. Some doctors think this is the best thing to prevent recurrence (1cm is what they aim for), I had 5mm of clear margins, which is shy of 5mm. But my surgeon was only looking for 2mm, and I have read that he has great success with this margin.

IDC Grade 2...I didn't realize I had DCIS until after my lumpectomy. I don't remember seeing a grade for it. I'm going to have to pull my path report out.
edited: just pulled out the report, it says DCIS is low grade and 8 mm.
margins clear by 1 cm or more and no evidence of lymphatic invasion.

Hi. My DCIS (which was the way more extensive component) was grade 3. My IDC was grade 2. By the way, I am also ER+ (though weakly), PR-, and HER2 + (strongly).

Just got my path report back. My IDC was 2.3 cm, grade 2. My DCIS was 1.3 cm, grade 3.

thanks for eveyones replies. Seems alot are (IDC w/DCIS) the same grade...But many are different grades too.

I had IDC and DCIS, separate quadrants. Not sure of the grades though and don't have copies of my original pathology from either. Only path report I have a copy of is the one done after my mastectomy.

You are absolutely right it does... Today I decided to sort through my stuff in more detail and found all the path reports including the one after my mastectomy and there it was. So... DCIS was grade 2 according to my biopsy on the microcalcificatins and the IDC also grade 2. I found reading that stuff more interesting now that I understand it all. What is says exactly is this " invasive ductal carcinoma, grade 2 of 3 tubule formation 3, nuclear pleomorphism 2, mitosis 1 "and actually the only path i dont have a copy of is the one done by the cancer center I am being treated at now. The original was done at Henry Ford and I am with St. John now. This report is the one that shows me her2 +..

I sure am ! St. John's is in Detroit right on the Grosse Pointe border. VanElslander Cancer Center is there and that is where I am being treated. I totally understand the fog btw

My IDC was 2/3, and my DCIS was HIgh Grade. There's no number. What does all that mean? I'm only interested in the 87% likelyhood of no recurrence with standard chemo and radiation. I'm starting T/H in two weeks. I hope it's less toxic than the A/C.

J'aime, it sounds like you IDC was grade 2 of 3 possible (or mid) and the DCIS was 3 of 3 or high grade. Hope you have less SE's from the T/H. Good Luck with that!
