    Report states Infiltrating ductal carcinoma, Nottingham grade II (of III ) tubules 3/3, nuclei 2/3, mitoses 1/3; Nottingham score 6/9 is identified forming a mass 1.8 x 1.6 x 1.0 cm, Ductal carcinoma in situ, low nuclear grade, comprises approximately 5-25% of the tumor volume, Angiolymphatic invasion absent. The non-neoplastic breast parenchyma shows nonproliferative fibrocystic changes. Biopsy site changes are present. All surgical resection marins, after a single separately submitted re-excision of the superior/deep margin are negative for tumor. My question for anyone that can answer it is can a tumor have IDC and SITU and does this seem like an agressive cancer??

    Mari-

    No, to my lay person knowledge, this does not seem like an agressive cancer. A grade 2 on the Nottingham is generally interpreted as a medium-grade. Grade 3s tend to be aggressive. When I was dxed 4yrs ago, my score was exactly like your's (tubles 3/3, nuclei 2/3 and mitosis 1/3). I called the patholigist who read my biopsy. He said, that in his view the mitosis or how quickly the cells are turning over is most important of the 3 and that they are thinking of revising the Nottingham to reflect this. the fact that yours was 1 our of 3 is a good sign. Your path also states no lymphangio invasion which means no cancer cells were found in the lymph or there were no cancer invasion to nearby blood vessels. - Another good sign.

    Also, have you looked into the oncotype test yet? It is for early stage/no lymph involvement and tests 21 genes for your particular tumor. I had it done and my score was low at 10, which was very helpful in deciding my tx. (No chemo, as anti-hormone therapy would be of more benefit.)

    About your other question, I only had IDC, but there are many ladies on these boards who have had both.

    By the way, my tumor size was pretty much the same as yours. I am doing well 4years out (now age 52) and you will too.

    Mari - Julie did a great job on the pathology, but I wanted to answer your question about whether you can have IDC and DCIS together.  And the answer is yes!   My Onc told it me is actually very common to find both forms,  because IDC starts inside the ducts (DCIS) and moves out.

    marie65, sorry you are here, but everyone will try to help you.  I had DCIS and IDC.  It was very small, but I did have both.  I was grade 2.  I am 6 months post chemo and coming up on my one year anniversary.  This journey will consume you, be ready.  Good luck and many good wishes.
